Efficacy of tirofiban combined with atorvastatin in patients with acute cerebral infarction undergoing mechanical thrombectomy

Abstract:Objective To investigate the clinical efficacy of tirofiban combined with atorvastatin in patients with acute cerebral infarction undergoing mechanical thrombectomy.Methods This study adopted a prospective randomized con-trolled trial design.A total of 112 patients with acute cerebral infarction undergoing mechanical thrombectomy admitted to Lu'an Hospital of Traditional Chinese Medicine from September 2022 to February 2025 were enrolled.Patients were randomly assigned to the control group(n=56)and the experimental group(n=56)using a random number table.The control group received conven-tional dual antiplatelet therapy combined with atorvastatin,while the experimental group received conventional dual antiplatelet therapy combined with tirofiban and atorvastatin.The treatment course lasted for 3 months.The clinical efficacy and adverse re-actions of patients in the control group and the experimental group,as well as changes in the Barthel Index(BI)scores,modified Rankin Scale scores,hemodynamic parameters(mean blood flow,mean blood flow velocity,end-diastolic velocity,peak systolic velocity),blood lipid levels(triglycerides,low-density lipoprotein cholesterol(LDL-C),total cholesterol,high-density lipopro-tein cholesterol(LDL-C)),and inflammatory factor levels(high-sensitivity C-reactive protein(CRP),interleukin-6)(IL-6)before and after treatment were observed and compared.Results The total effective rate in the experimental group was 89.29%(50/56),higher than that in the control group(73.21%(41/56)),and the difference was statistically significant(χ2=4.747,P<0.05).After treatment,the BI score in the experimental group was higher than that in the control group((78.62±18.50)vs(66.83±16.51)),with a statistically significant difference(t=3.558,P<0.05).The proportion of patients with a favorable prognosis(modified Rankin scale score of 0-2)in the experimental group was 73.21%(41/56),higher than that in the control group(44.64%(25/56)),and the difference was statistically significant(χ2=9.444,P<0.05).After treatment,the levels of mean blood flow velocity,mean blood flow volume,end-diastolic velocity,and peak systolic velocity in the experimental group((58.25±4.27)cm/s,(9.53±1.38)mL/s,(67.73±5.83)cm/s,(112.67±11.47)cm/s,respectively)were all higher than those in the control group((54.36±4.37)cm/s,(8.24±1.08)mL/s,(58.52±5.32)cm/s,(99.52±10.35)cm/s,respectively)(t=4.764,5.509,8.733,6.370,all P<0.05).After treatment,the levels of LDL-C,triglycerides,and total cholesterol in the experimental group((2.03±0.47)mmol/L,(0.87±0.21)mmol/L,(2.73±0.74)mmol/L,respectively)were all lower than those in the control group((2.87±0.54)mmol/L,(1.15±0.28)mmol/L,(3.62±0.96)mmol/L,respectively)(t=8.781,5.987,5.495,all P<0.05),and the HDL-C level was significantly higher than that in the control group((1.75±0.38)mmol/L vs(1.36±0.46)mmol/L),with a statis-tically significant difference(t=4.891,P<0.05).After treatment,the levels of hypersensitive CRP and IL-6 in the experi-mental group((5.62±1.63)mg/L,(6.42±1.27)ng/L,respectively)were lower than those in the control group((7.53±2.01)mg/L,(7.42±1.50)ng/L,respectively)(t=5.523,3.807,both P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(16.07%(9/56)vs 12.50%(7/56))(χ2=0.624,P=0.430).Conclusion Atorvastatin combined with tirofiban demonstrates superior clinical efficacy in treating acute cerebral infarction undergoing mechanical thrombectomy,improves patients'quality of life,regulates blood lipid levels,reduces inflammatory responses,enhances hemodynamics,and exhibits good safety.
