Meta-analysis of doxofylline as an adjuvant therapy for middle-aged and elderly patients with acute exacerbation of chronic obstructive pulmonary disease

Abstract：Objective To systematically evaluate the efficacy of doxofylline as an adjuvant therapy in middle-aged and elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)by using the method of Meta-analysis.Methods Clinical studies on doxofylline as an adjuvant therapy in middle-aged and elderly patients with AE-COPD were collected from PubMed,the Cochrane Library,Web of Science,China National Knowledge Infrastructure(CNKI),Wanfang Data Knowledge Service Platform and VIP Chinese Science and Technology Periodical Database,with publication deadline set in September 2024.After independently screening by two researchers using inclusion and exclusion criteria,the literature data were analyzed by Meta-analysis using Review Manager 5.3 software(version 5.3).Results Among sixteen included studies,the total sample size was 1 201 cases.The results of Meta-analysis showed that compared with the control group,the effective rate of doxofylline adjuvant therapy in patients with AECOPD was increased(RR=1.24,95%CI=(1.17,1.32),P<0.05),and the lung function indicators of the patients including the forced expiratory volume in one second(FEV1)of the patients(SMD=1.48,95%CI=(1.33,1.63),P<0.05),forced vital capacity(FVC)(SMD=1.86,95%CI=(1.43,2.28),P<0.05),FEV1/FVC(SMD=1.00,95%CI=(0.76,1.24),P<0.05),partial pressure of arterial oxygen(PaO2)(SMD=0.95,95%CI=(0.79,1.11),P<0.05),partial pressure of arterial carbon dioxide(PaCO2)(SMD=-0.97,95%CI=(-1.30,-0.65),P<0.05),arterial ox-ygen saturation(SaO2)(SMD=1.06,95%CI=(0.36,1.76),P<0.05)and peak expiratory flow(PEF)(SMD=1.05,95%CI=(0.48,1.62),P<0.05)were all improved.Conclusion The results of the meta-analysis suggest that doxofylline,as an adjuvant ther-apy,may have a good therapeutic effect on middle-aged and elderly patients with AECOPD,but its application still requires cautious consideration.
