Application of combined anesthesia with ciprofol and remifentanil in painless induced abortion for obese patients

Abstract:Objective To analyze the application of combined anesthesia with ciprofol and remifentanil in painless induced abortion(PIA)for obese patients.Methods A total of 124 obese patients who underwent PIA in Tangshan Maternal and Children Health Hospital from January 2023 to June 2024 were divided into the propofol group(62 cases)and the ciprofol group(62 cases)according to the random number table method.The propofol group received propofol combined with remifen-tanil anesthesia,while the ciprofol group received ciprofol combined with remifentanil anesthesia.The operation time,postop-erative recovery time,adverse reactions,hemodynamic indicators at different time points,respiratory-related adverse events and analgesic effect were observed in the two groups.Results The excellent and good anesthesia rate in the ciprofol group was 93.55%(58/62),significantly higher than that in the propofol group(84.65%(50/62))(χ2=4.593,P=0.032).The eye-opening time and hospital discharge time in the ciprofol group were shorter than those in the propofol group(eye-opening time:(4.66±1.11)min vs(7.89±2.49)min,discharge time:(12.02±3.32)min vs(22.14±5.63)min),and the differences were statistically sig-nificant(t=-20.114,-22.631,both P<0.05).There were statistically significant differences in the time main effect,group main effect and interaction effect of mean arterial pressure(MAP),heart rate(HR)and pulse oxygen saturation(SpO2)between the two groups(Ftime=421.070,398.181,257.177,Fgroup=15.380,18.762,14.616,Finteraction=12.338,9.869,11.060,all P<0.05).The MAP,HR and SpO2 at T1 and T2 time points in the ciprofol group(T1 time point:(76.66±3.63)mmHg(1 mmHg=0.133 kPa),(75.25±4.02)beats/min,(96.55±0.30)%,T2 time point:(76.14±3.25)mmHg,(74.69±3.89)beats/min,(96.50±0.29)%)were high-er than those in the propofol group(T1 time point:(72.74±4.11)mmHg,(70.44±4.85)beats/min,(96.22±0.29)%,T2 time point:(71.36±3.8)mmHg,(69.62±4.65)beats/min,(96.13±0.27)%),and the differences were statistically significant(t=5.629,6.012,6.227,7.470,6.585,7.353,all P<0.05).The Visual Analogue Scale scores for pain at 10 minutes and 30 minutes after surgery in the ciprofol group were lower than those in the propofol group(10 minutes after surgery:(1.33±0.36)points vs(2.10±0.58)points,30 minutes after surgery:(2.36±0.64)points vs(5.58±1.44)points),and the differences were statistically significant(t=-8.882,-16.090,both P<0.05).The incidence rates of respiratory depression and hypoxemia in the ciprofol group were 8.06%(5/62)and 9.68%(6/62),respectively,significantly lower than those in the propofol group(25.81%(16/62)and 29.03%(18/62)),and the differences were statistically significant(χ2=6.937,7.440,P=0.008,0.006).The incidence of hypotension in the ciprofol group was 6.45%(4/62),lower than that in the propofol group(22.58%(14/62))(χ2=6.499,P=0.011).Conclusion The combi-nation of ciprofol and remifentanil anesthesia has a better anesthetic effect in obese patients undergoing PIA,which can not only shorten the eye-opening time after surgery and hospital discharge time,reduce the degree of postoperative pain but also maintain more stable hemodynamics during surgery.In addition,it can effectively reduce the incidence of respiratory-related adverse events,thereby enhancing overall safety.
