Clinical evaluation of aspirin combined with low molecular weight heparin calcium in pregnant women with preeclampsia

Abstract:Objective To explore the effect of aspirin combined with low molecular weight heparin calcium on thromboelastogram and pregnancy outcomes of pregnant women with high-risk factors for preeclampsia.Methods A total of 80 pregnant women with high-risk factors for preeclampsia admitted to Shijiazhuang Maternal and Child Health Hospi-tal from June 2022 to June 2024 were enrolled in the study and randomly divided into the control group(40 cases,treated with conventional intervention and magnesium sulfate injection)and the observation group(40 cases,treated with aspirin combined with low molecular weight heparin calcium on the basis of the control group)by the random number table meth-od.The incidence of preeclampsia,thromboelastogram(maximum amplitude(MA),clotting response time(R),coagula-tion angle(α),thrombin formation time(K),coagulation composite index(CI)),uterine hemodynamics(pulsatility index(PI),resistance index(RI))before and after the treatment and pregnancy outcomes(placental abruption,premature deliv-ery,postpartum hemorrhage,neonatal asphyxia)were compared between the two groups.Results The incidence of pre-eclampsia in the observation group was lower than that in the control group after the treatment(2.50%(1/40)vs 17.50%(7/40),χ2=5.000,P<0.05).The levels of R and K were higher in the observation group than those in the control group((6.13±1.05)min,(1.89±0.43)min vs(5.41±1.07)min,(1.61±0.33)min),while the levels of α angle,MA and CI were low-er in the observation group than those in the control group((51.34±2.37)°,(53.14±2.06)mm,(0.89±0.31)vs(55.48±2.11)°,(56.15±1.56)mm,(1.05±0.28)),and there were significant differences between the two groups(t=3.038,3.267,8.252,7.367,2.422,all P<0.05)).The levels of RI and PI were lower in the observation group than those in the control group((0.31±0.05),(0.50±0.08)vs(0.38±0.06),(0.59±0.09)),and there were significant differences between the two groups(t=5.668,4.727,both P<0.05).The incidence rates of placental abruption,premature delivery,postpartum hemorrhage and neonatal asphyxia were lower in the observa-tion group than those in the control group(0.00%(0/40)vs 10.00%(4/40),5.00%(2/40)vs 20.00%(8/40),2.50%(1/40)vs 15.00%(6/40),0.00%(0/40)vs 12.50%(5/40)),and there were significant differences between the two groups(χ2=4.211,4.114,3.914,5.333,all P<0.05).Conclusion Aspirin combined with low molecular weight heparin calcium may reduce the incidence of preeclampsia by reg-ulating coagulation function and improving uterine blood flow,thereby reducing the incidence of adverse pregnancy outcomes.
