The preventive effect of low molecular weight heparin calcium in combination with intermittent pneumatic compression on venous thrombosis after laparoscopic radical gastrectomy in gastric cancer patients

Abstract：Objective To investigate the preventive effect of low molecular weight heparin calcium(LMWH-Ca)in combination with intermittent pneumatic compression(IPC)on venous thrombosis after laparoscopic radical gastrectomy ingastric cancer patients.Methods A total of 62 patients who underwent laparoscopic radical gastrectomy for gastric cancer ad-mitted to the Affiliated Hospital of Traditional Chinese Medicine of Nanyang Medical College from February 2022 to February 2025 were prospectively included and divided into the LMWH-Ca group(31 cases)and the IPC group(31 cases)by random number table method.The IPC group was treated with IPC,and the LMWH-Ca group was treated with LMWH-Ca on the basis of the IPC group.Both the LMWH-Ca group and the IPC group were treated for one week.The incidence of venous thrombo-sis during treatment,coagulation function,inflammatory factors,hemodynamic indicators before treatment and one week after treatment,as well as other adverse reactions during treatment were compared between the LMWH-Ca group and the IPC group.Results The incidence of deep vein thrombosis was 0.00%(0/31)in the LMWH-Ca group and 19.35%(6/31)in the IPC group,and the difference was statistically significant(χ²=4.613,P<0.05).After one week of treatment,the levels of D-dimer and fibrinogen in the LMWH-Ca group and the IPC group were lower than those before treatment.Moreover,these indicators in the LMWH-Ca group((0.47±0.12)μg/mL and(3.13±0.76)g/L,respectively)were lower than those in the IPC group((0.69±0.17)μg/mL and(4.35±0.76)g/L,respectively)(t=5.887,6.320,both P<0.05).After one week of treatment,the prothrombin time and activated partial prothrombin time in the LMWH-Ca group and the IPC group were prolonged compared with those before treatment,and these indicators in the LMWH-Ca group((12.31±1.79)s and(30.35±3.61)s,respectively)were significantly longer than those in the IPC group((10.73±1.88)s and(27.02±3.31)s,respectively)(t=3.408,3.786,both P<0.05).After one week of treatment,the levels of serum interleukin-6,tumor necrosis factor-α,and soluble myeloid cell triggering receptor 1 in the LMWH-Ca group and the IPC group were lower than those before treatment.Moreover,these indicators in the LMWH-Ca group((14.31±3.11)ng/L,(10.46±2.13)ng/L and(34.85±5.30)ng/L,respectively)were all lower than those in the IPC group((17.66±3.15)ng/L,(12.50±3.11)ng/L and(41.43±5.71)ng/L,respectively)(t=4.214,3.013,4.703,all P<0.05).After one week of treatment,maximum venous outflow,venous capacitance,and venous refilling time increased in the LMWH-Ca group and the IPC group increased compared with that before treatment,and these indicators in the LMWH-Ca group((6.61±0.72)mL,(7.48±1.05)mL,(12.20±1.24)s)were significantly higher than those in the IPC group((5.35±0.75)mL,(6.08±1.25)mL,(10.60±1.45)s)(t=6.748,4.775,4.669,all P<0.05).The incidence of adverse reactions was 12.90%(4/31)in the LMWH-Ca group and 6.45%(2/31)in the IPC group,with no statistically significant difference between the LMWH-Ca group and the IPC group(χ²=0.185,P>0.05).Conclusion The combination of LMWH-Ca and IPC in patients after laparoscopic radical gastrec-tomy can reduce the risk of venous thrombosis,inhibit the expression of inflammatory factors,regulate coagulation function and hemodynamic indicators,and has good safety.
