Effects of tranexamic acid and epsilon-aminocaproic acid on blood loss and the level of inflammation in patients undergoing thoracolumbar fracture surgery

Abstract:Objective To explore the effects of tranexamic acid(TXA)and epsilon-aminocaproic acid(EACA)on blood loss and the level of inflammation in patients undergoing thoracolumbar fracture surgery.Methods A total of 120 patients undergoing thoracolumbar fracture surgery admitted to Suqian Hospital of Nanjing Drum-Tower Hospital Group from June 2023 to June 2025 were selected and divided into the TXA group and the EACA group using a random number table meth-od,with 60 patients in each group.The TXA group was treated with TXA,and the EACA group was treated with EACA.The intraoperative blood loss,wound blood drainage volume and other related indicators were compared between the TXA group and the EACA group,along with inflammatory markers at 24 hours and 72 hours after the operation.Results The wound blood drainage volume in the TXA group((186.43±55.62)mL)was lower than that in the EACA group((215.62±63.48)mL)(t=2.679,P=0.008).At 24 hours after the operation,the hemoglobin and hematocrit in the TXA group((113.64±19.48)g/L and(33.46±3.76)%,respectively)were higher than those in the EACA group((106.53±18.37)g/L and(31.52±3.52)%,respectively)(t=2.057,2.918,P=0.042,0.004).At 24 hours after the operation,the white blood cell count,the ratio of neutrophils and the levels of C-reactive protein(CRP),tumor necrosis factor-α,and interleukin-6(IL-6)in the TXA group((14.53±2.34)×109/L,(82.42±5.15)%,(12.52±1.51)mg/L,(1.61±0.18)pg/mL and(11.25±2.16)pg/mL,respectively)were lower than those in the EACA group((15.64±3.15)×109/L,(84.34±3.48)%,(13.06±1.24)mg/L,(1.68±0.12)pg/mL and(12.12±2.20)pg/mL,respec-tively)(t=2.191,2.393,2.141,2.506,2.186,P=0.030,0.018,0.034,0.014,0.031).At 72 hours after the operation,the ratio of neutrophil and the levels of CRP and IL-6 in the TXA group((81.02±3.42)%,(9.32±1.28)mg/L and(9.92±2.11)pg/mL,respec-tively)were lower than those in the EACA group((82.51±3.69)%,(10.01±1.06)mg/L and(10.78±2.14)pg/mL,respectively)(t=2.294,3.216,2.217,P=0.024,0.002,0.029).Conclusion Compared with EACA,TXA demonstrates superior postoperative hemostatic stability and anti-inflammatory effects in posterior thoracolumbar fracture surgery.
