Clinical evaluation of Jide Sheng Snake Medicine tablets for oral administration combined with external application in treating pit viper bites

Abstract：Objective To explore the effect of Jide Sheng Snake Medicine tablets for oral administration combined with external application on swelling and local inflammatory response after pit viper bites based on conventional wound debridement.Methods A total of 108 patients with viper bites admitted to Hanzhong Central Hospital from September 2017 to May 2023 were retrospectively analyzed.The patients were divided into the control group(n=44)and the observation group(n=64)based on different treatment methods.The observation group received conventional therapy plus oral administration and ex-ternal application of Jide Sheng Snake Medicine tablets,while the control group received conventional therapy plus oral ad-ministration of Jide Sheng Snake Medicine tablets.Both groups underwent standard wound debridement and anti-infective treatment,and the treatment course lasted 7 days.The clinical efficacy of the two groups of patients was evaluated,and the changes of symptoms(pain intensity and swelling)before treatment and 7 days after treatment were observed.The changes of blood routine index,coagulation function index and inflammation-related index before treatment and 7 days after treat-ment were compared.Results The cure rate of the observation group was 57.81%(37/64),higher than 36.36%(16/44)of the control group(χ2=4.800,P<0.05).After the treatment for 3,5,and 7 days,the pain scores of the observation group were re-spectively(4.37±0.49)points,(3.51±0.42)points and(2.87±0.31)points,lower than(4.69±0.67)points,(3.79±0.59)points and(3.04±0.45)points of the control group,and the swelling values of the observation group were respectively(30.21±6.43)mm,(15.53±3.94)mm and(9.27±3.66)mm,lower than(34.04±6.81)mm,(18.33±5.17)mm and(11.43±4.24)mm of the con-trol group.The differences were all statistically significant(tpain=2.867,2.882,2.326,tswelling=2.969,3.192,2.824,all P<0.05).The blood platelet count and peripheral blood fibrinogen levels of the two groups were respectively((225.84±45.03)×109/L,(3.57±0.52)g/L,(206.63±51.34)×109/L and(3.32±0.68)g/L)after the treatment for 7 days,higher than((183.63±60.34)×109/L,(2.63±0.84)g/L,(181.58±64.30)×109/L and(2.61±0.90)g/L)before the treatment,and the levels of the above indexes of the observation group were higher than those of the control group(t=2.057,2.163,both P<0.05).The levels of neutrophil/lym-phocyte ratio,platelet/lymphocyte ratio,thrombin time,peripheral blood D-dimer,interleukin-1,interleukin-6 and prostaglan-din E2 of the two groups were respectively((31.88±1.64)%,(14.07±6.67)%,(15.28±1.71)s,(296.97±36.02)μg/L,(39.88±5.77)pg/mL,(49.43±9.38)pg/mL,(214.97±16.02)μg/L,(33.09±2.08)%,(18.39±7.09)%,(16.33±1.64)s,(320.12±49.81)μg/L,(43.51±6.31)pg/mL,(53.97±10.06)pg/mL and(222.12±19.86)μg/L)after the treatment for 7 days,lower than((87.86±2.62)%,(44.33±9.57)%,(17.86±3.12)s,(451.63±70.61)μg/L,(71.44±9.12)pg/mL,(107.27±11.34)pg/mL,(297.47)±20.29)μg/L,(88.74±2.90)%,(46.48±12.60)%,(18.74±3.35)s,(462.58±78.30)μg/L,(73.95±10.35)pg/mL,(110.58±13.30)pg/mL and(293.11±23.82)μg/L)before the treatment,and the levels of the above indexes of the observation group were lower than those of the control group(t=3.374,3.223,3.213,2.804,3.092,2.399,2.065,all P<0.05).Conclusion Based on conventional wound de-bridement,oral administration and external application of Jide Sheng Snake Medicine tablets can quickly relieve the limb swelling and pain in treatment of patients with viper bites and improve the body's coagulation function and inflammatory response,and its application effect is better than the oral administration alone.
