Efficacy of donepezil combined with citicoline in the treatment of cognitive impairment after hypertensive cerebral infarction

Abstract:Objective To investigate the clinical efficacy of donepezil combined with citicoline in the treatment of cognitive impairment after hypertensive cerebral infarction.Methods A prospective randomized controlled study was con-ducted.A total of 62 patients with cognitive impairment after hypertensive cerebral infarction who were admitted to Luoyang Petrochemical Hospital from January 2022 to December 2024 were selected and divided into the observation group(treated with donepezil combined with citicoline,n=31)and the control group(treated with citicoline alone,n=31)according to the random number table method.The cognitive status,cognitive function,daily living ability,clinical efficacy and inflamma-tory factors levels before treatment and after 3 courses of treatment and adverse reactions in both groups were observed.Results After 3 courses of treatment,the National Institutes of Health Stroke Scale and Clinical Dementia Rating scores of the observation group and the control group((6.33±2.56)points,(0.98±0.33)points,(7.74±2.59)points,(1.26±0.29)points,respectively)were significantly lower than those before treatment((12.28±3.20)points,(1.60±0.24)points,(11.97±3.63)points,(1.65±0.21)points,respectively)(t=8.084,8.460,5.282,6.065,all P<0.05),and the above indicators in the observation group were lower than those in the control group(t=2.156,3.549,both P<0.05).After treatment,the scores of the Mini-Mental State Examination,Activities of Daily Living Scale and Montreal Cognitive Assessment in the observation group and the control group((25.12±4.08)points,(59.05±9.12)points,(24.59±4.95)points,(22.73±3.96)points,(53.77±8.03)points,(22.04±4.18)points,respectively)were significantly higher than those before treatment((16.67±3.31)points,(42.65±6.56)points,(16.56±3.71)points,(16.09±3.14)points,(44.15±6.94)points,(16.95±3.55)points,respectively)(t=8.955,8.128,7.227,7.315,5.047,5.168,all P<0.05),and the scores in the observation group were significantly higher than those in the control group(t=2.340,2.419,2.191,all P<0.05).The total effective rate in the observation group was significantly higher than that in the control group(93.55%(29/31)vs 74.19%(23/31)),and the difference was statistically significant(χ2=4.292,P<0.05).After treatment,the levels of interleukin-6 and serum C-reactive protein in the observation group and the control group((103.65±10.69)ng/L,(5.04±1.22)mg/L,(109.85±11.81)ng/L,(6.96±1.68)mg/L,respectively)were significantly lower than those before treatment((134.10±16.27)ng/L,(13.08±3.65)mg/L,(133.58±15.86)ng/L,(12.65±3.45)mg/L,respectively),and the differences were statistically significant(t=8.709,11.632,6.682,8.256,all P<0.05),and the above in-dicators in the observation group were lower than those in the control group(t=2.167,5.149,both P<0.05).The incidence of adverse reactions in the two groups(12.90%(4/31)vs 19.35%(6/31))was not statistically significant(χ2=0.477,P=0.490).Conclusion The combination of donepezil and citicoline in the treatment of cognitive impairment after hypertensive cere-bral infarction can effectively alleviate cognitive impairment,promote the recovery of cognitive function,improve daily liv-ing abilities,and control inflammatory responses.
