Efficacy of nalfurafine combined with loratadine in the treatment of chronic kidney disease-associated pruritus

Abstract:Objective To investigate the clinical efficacy and safety of nalfurafine combined with loratadine in the treatment of chronic kidney disease-associated pruritus(CKD-aP).Methods A total of 114 patients with CKD-aP who were treated at Nanyang First People's Hospital from December 2023 to June 2025 were selected and randomly divided into the ob-servation group and the control group by the random number table method,with 57 cases in each group.The control group was treated with loratadine,while the observation group was treated with nalfurafine combined with loratadine.The clinical efficacy,pruritus severity(Visual Analogue Scale(VAS)score and modified Duo Pruritus Scale score),sleep quality(Pittsburgh Sleep Quality Index(PSQI)score),the levels of inflammatory factors(interleukin-6(IL-6)and C-reactive protein(CRP)),and the in-cidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was 92.98%(53/57),higher than 77.19%(44/57)in the control group(χ2=5.600,P<0.05).After treatment,the VAS score,modi-fied Duo Pruritus Scale score and PSQI score in the observation group((2.56±1.31)points,(11.88±3.25)points and(5.08±1.63)points,respectively)were lower than those in the control group((3.25±1.27)points,(15.47±3.67)points and(8.14±2.11)points,respectively)(t=2.855,5.529,8.665,all P<0.05).After treatment,the levels of IL-6 and CRP in the observation group((15.45±2.68)pg/mL and(10.07±1.82)mg/L,respectively)were lower than those in the control group((18.24±3.23)pg/mL and(13.11±2.25)mg/L,respectively)(t=5.019,7.931,both P<0.05).The incidence of adverse reactions in the two groups were 12.28%(7/57)and 10.53%(6/57),respectively,with no statistically significant difference(χ2=0.087,P=0.768).Conclusion The combination of nalfurafine and loratadine has a definite therapeutic effect on CKD-aP,effectively alleviating pruritus severity,improving sleep quality,reducing the levels of inflammatory factors,with good safety.
