Efficacy of dapagliflozin combined with metoprolol in the treatment of valvular heart disease complicated with heart failure

Abstract:Objective To explore the efficacy of dapagliflozin combined with metoprolol in treating valvular heart disease complicated with heart failure and its influence on cardiac function and serum brain natriuretic peptide(BNP).Methods A total of 124 patients with valvular heart disease complicated with heart failure admitted to Anyang Sixth Peo-ple's Hospital from January 2021 to March 2024 were retrospectively selected.The 60 patients treated with metoprolol were assigned to the control group,and the 64 patients treated with dapagliflozin combined with metoprolol were assigned to the observation group.The clinical efficacy of the two groups after 6 months of treatment,cardiac function indicators,myocardial markers(cardiac troponin I(cTnI)and BNP)before and after treatment,and the incidence of adverse reactions during treatment were compared between two groups.Results The total effective rate of the control group and the observation group were 78.33%(47/60)and 93.75%(60/64),respectively,with a statistically significant difference(χ2=6.222,P=0.013).After 6 months of treatment,the left ventricular ejection fraction(LVEF)in both groups increased,while the left ventricular end-diastolic di-ameter(LVEDD)and left ventricular end-systolic diameter(LVESD)decreased,and the LVEF,LVEDD and LVESD of the observation group((58.45±4.67)%,(46.34±3.24)mm,(41.34±3.72)mm)were better than those of the control group((52.62±4.21)%,(52.86±3.70)mm,(49.02±4.41)mm)(t=7.285,10.456,10.505,all P<0.001).After 6 months of treatment,serum BNP and cTnI levels decreased in both groups,and the serum BNP and cTnI in the observation group((175.03±15.85)μg/L,(0.28±0.07)ng/mL)were both lower than those in the control group((243.52±17.14)μg/L,(0.56±0.14)ng/mL)(t=23.118,14.219,both P<0.001).The incidence of adverse reactions in the control group and the observation group was 23.33%(14/60)and 26.56%(17/64),respectively,with no significant difference(χ2=0.172,P=0.678).Conclusion Dapagliflozin combined with metoprolol can enhance the clinical efficacy of patients with valvular heart disease and heart failure,effectively improve left ven-tricular ejection function and reduce ventricular volume load,while significantly lowering serum BNP and cTnI levels,without increasing adverse reactions.
