Efficacy of vericiguat combined with metoprolol in the treatment of elderly patients with heart failure and arrhythmia

Abstract：Objective To explore the efficacy of vericiguat combined with metoprolol in the treatment of elder-ly patients with heart failure and arrhythmia and its influence on brain natriuretic peptide(BNP)and 6-minute walk distance(6MWD).Methods A total of 102 elderly patients with heart failure and arrhythmia treated at Suzhou Ninth People's Hospital from August 2022 to August 2024 were randomly divided into a control group and an observation group(51 cases each)us-ing a random number table method.The control group was treated with basic medication and metoprolol,and the observation group was treated with basic medication plus metoprolol and vericiguat.Both groups were treated for 3 months.The efficacy,BNP,left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),6MWD,stroke volume,left ventricular ejection fraction(LVEF),and the adverse reactions during treatment were compared between the two groups.Results The total effective rate of the observation group was 90.20%(46/51),higher than that in the control group(70.59%(36/51))(χ2=6.220,P=0.013).After treatment,the serum levels of BNP decreased and the 6MWD increased in both groups of patients.Moreover,the above indicators in the observation group((712.60±64.13)pg/mL and(358.47±32.26)m,respectively)were better than those in the control group((893.42±80.41)pg/mL and(309.26±27.81)m,respectively)(χ2=12.555,8.251,both P<0.001).After treatment,LVEDD and LVESD decreased,while stroke volume and LVEF increased in both groups.The above indicators were observed in the observation group((60.42±2.44)mm,(51.20±2.56)mm,(4.54±0.81)L/min,(50.96±2.92)%,respectively)were better than those in the control group((61.99±3.05)mm,(53.54±2.67)mm,(4.13±0.73)L/min,(46.73±2.33)%,respectively)(t=2.871,4.518,2.685,8.086,P=0.005,0.001,0.008,0.001).The incidence of adverse reactions was 5.88%(3/51)in the control group and 9.80%(5/51)in the observation group,with no significant difference(χ2=0.136,P=0.713).Conclusion The combination of vericiguat and metoprolol can achieve better clinical efficacy in elderly patients with heart failure and arrhythmia,effectively improve cardiac function indicators,reduce serum BNP levels and prolong 6MWD without increasing adverse reactions,which is a safe and effective combination treatment.
