Efficacy of different doses of semaglutide combined with high-protein diet on the treatment of type 2 diabetes mellitus with obesity

Abstract:Objective To investigate the clinical efficacy of different doses of semaglutide combined with high-pro-tein diet on the treatment of type 2 diabetes mellitus(T2DM)complicated with obesity.Methods A total of 416 patients with T2DM complicated with obesity treated at Hengshui People's Hospital from July 2022 to December 2024 were prospectively se-lected and randomly divided into 4 groups according to the simple randomization method.Group A was treated with metformin plus high-protein diet,Group B was treated with the low-dose semaglutide plus high-protein diet,Group C was treated with the medium-dose semaglutide plus high-protein diet group,and Group D was treated with the high-dose semaglutide plus high-pro-tein diet group,with 104 patients in each group.The clinical efficacy,blood glucose and metabolism indicators,blood lipid metabolism indicators,body mass index(BMI)and adverse reactions were compared among the four groups before and after the treatment.Results After 4 months of treatment,the total effective rates for Group A,B,C,and D were(69.23%(72/104)vs 73.08%(76/104)vs 82.69%(86/104)vs 91.35%(95/104)),with a statistically significant difference(χ2=18.647,P<0.05).The following indicators exhibited a graded increase across the four groups:fasting blood glucose(Group D(5.09±1.24)mmol/L<Group C(6.27±1.27)mmol/L<Group B(6.58±1.26)mmol/L<Group A(7.47±1.28)mmol/L),2-hour postprandial blood glucose(Group D(7.31±1.28)mmol/L<Group C(8.49±1.45)mmol/L<Group B(8.75±1.67)mmol/L<Group A(9.36±2.11)mmol/L),and glycated hemoglobin(Group D(6.09±0.36%)<Group C(6.38±0.23%)<Group B(6.56±0.24%)<Group A(6.79±0.29%))(F=63.092,27.985,112.105,all P<0.05).The following indicators exhibited statistically significant graded increases across the four groups:total cholesterol(Group D(3.07±0.65)mmol/L<Group C(3.79±1.14)mmol/L<Group B(4.12±1.12)mmol/L<Group A(4.82±0.71)mmol/L),triglycerides(Group D(1.12±0.41)mmol/L<Group C(1.39±0.52)mmol/L<Group B(1.56±0.61)mmol/L<Group A(2.79±0.59)mmol/L),low density lipoprotein cholesterol(Group D(2.23±0.42)mmol/L<Group C(2.69±0.39)mmol/L<Group B(2.88±0.51)mmol/L<Group A(3.79±0.65)mmol/L)(F=63.178,196.182,179.296,all P<0.05).The high density lipoprotein choles-terol levels showed a statistically significant decreasing trend from Group D to A:Group D(1.59±0.21)mmol/L>Group C(1.38±0.23)mmol/L>Group B(1.16±0.19)mmol/L>Group A(1.07±0.22)mmol/L(F=124.533,P<0.05).The BMI showed statistically significant differences across the four groups,with Group D(21.67±1.35)kg/m2<Group C(23.22±1.43)kg/m2<Group B(24.31±1.58)kg/m2<Group A(25.39±1.68)kg/m2(F=114.247,P<0.05).Meanwhile,the incidence of adverse reactions among groups A,B,C and D showed no statistically significant difference(5.77%(7/104)vs 6.73%(8/104)vs 7.69%(7/104)vs 6.73%(6/104))(χ2=0.306,P>0.05).Conclusion The combination of semaglutide and high-protein diet has a definite effect on T2DM with obesity.The combination of high-dose semaglutide and high-protein diet is more effective in improving the treatment efficacy,reducing blood glucose and blood lipids,and improving BMI,without increasing the risk of adverse reactions.
