Abstract:Objective To investigate the efficacy of insulin degludec and insulin aspart combined with teneligliptin in the treatment of insulin-resistant type 2 diabetes mellitus(T2DM).Methods A total of 116 patients with insulin-resistant T2DM treated in the Third Affiliated Hospital of Xinxiang Medical University from January 2023 to December 2024 were selected and divided into the control group and the observation group by random number table method. The control group(58 patients)received basic treatment combined with teneligliptin, and the observation group(58 patients)received insulin degludec and insulin aspart in addition to the control group treatment. Both groups were treated for 3 months. The efficacy, the levels of blood glucose indexes, blood lipid indicators, pancreatic islet function indicators, and the incidence of adverse reactions between the control group and the observation group were compared. Results The total effective rate was 94.83%(55/58)in the observation group, higher than 79.31%(46/58)in the control group(χ2=6.202,P<0.05).After 3 months of treatment, the fasting blood glucose,2-hour postprandial blood glucose and glycated hemoglobin levels in the observation group((6.24±0.71)mmol/L,(7.46±0.52)mmol/L and(5.61±0.54)%,respectively)were lower than those in the control group((7.06±0.89)mmol/L,(8.12±0.65)mmol/L and(6.13±0.71)%,respectively)(t=5.485,6.038,4.440,all P<0.05).After 3 months of treatment, the total cholesterol, triglyceride and low density lipoprotein cholesterol levels in the observation group((4.22±0.31)mmol/L,(1.25±0.35)mmol/L and(2.32±0.41)mmol/L,respectively)were lower than those in the control group((4.57±0.45)mmol/L,(1.69±0.38)mmol/L and(2.83±0.55)mmol/L,respectively)(t=4.878,6.486,5.662,all P<0.05).After 3 months of treat-ment,the fasting insulin level and insulin resistance index in the observation group((10.04±1.22)μU/L and(2.79±0.47),respectively)were lower than those in the control group((11.61±1.31)μU/L and(3.64±0.52),respectively),and the pancreatic β-cell function index in the observation group(60.12±7.29)was higher than that in the control group(56.34±6.93)(t=6.652,9.235,2.862,all P<0.05).The total incidence of adverse reactions in the observation group and the control group(8.62%(5/58)vs 10.34%(6/58))showed no significant difference(χ2=0.100,P>0.05).Conclusion The combination of insulin degludec and insulin aspart and teneligliptin has a defi-nite therapeutic effect on patients with insulin-resistant T2DM,demonstrating stable blood glucose and lipid levels, enhancing insulin sensitivity, and exhibiting favorable safety profiles.
