Influence of continuous blood glucose monitoring combined with insulin pump on pregnant women of advanced maternal age with gestational diabetes mellitus

Abstract:Objective To observe the effect of continuous blood glucose monitoring system(CGMS)combined with continuous subcutaneous insulin infusion(CSII)on improving blood glucose level and pregnancy outcome in pregnant women of advanced maternal age with gestational diabetes mellitus(GDM).Methods A total of 90 pregnant women of advanced maternal age with GDM who received prenatal check-ups at Baoji Hospital of Traditional Chinese Medicine from January 2021 to January 2024 were selected and randomly divided into the observation group and the control group(45 cases in each group)using a ran-dom number table method.The control group received multi-point blood glucose monitoring combined with CSII,while the ob-servation group received CGMS combined with CSII.The blood glucose levels,blood glucose target time,the dosage of insulin,the incidence of hypoglycemia,serum adipokine levels,and clinical pregnancy outcomes were compared between the two groups.Results After treatment,the levels of fasting blood glucose(FBG),2-hour postprandial blood glucose(2hPG)and glycosylated hemoglobin(HbA1c)in the observation group decreased compared with those before treatment,and the levels of FBG,2hPG and HbA1c in the observation group((5.02±0.51)mmol/L,(7.15±1.09)mmol/L,(6.71±0.42)%)were lower than those in the control group((5.45±0.68)mmol/L,(7.86±1.26)mmol/L,(7.09±0.56)%)(t=3.394,2.859,3.642,all P<0.05).The blood glucose target time,the dosage of insulin and the incidence of hypoglycemia in the observation group((11.15±2.37)d,(0.81±0.13)U/(kg·d),(0.48±0.25)times per person)were lower than those in the control group((14.51±3.37)d,(0.87±0.11)U/(kg·d),(0.71±0.28)times per person)(t=5.471,2.364,4.110,all P<0.05).After treatment,the levels of visfatin,vaspin and chemerin in both groups decreased compared with those before treatment,and the levels of of visfatin,vaspin and chemerin in the observation group((6.02±1.15)ng/mL,(25.33±3.36)ng/mL,(6.73±0.61)ng/mL)were lower than those in the control group((6.67±1.18)ng/mL,(29.09±3.42)ng/mL,(7.05±0.53)ng/mL)(t=2.246,5.261,2.656,all P<0.05).After treatment,the quality of life questionnaire for women with Gestational Diabetes Mellitus Quality(GDMQ)and General Comfort Questionnaire(GCQ)scores in the observation group((80.02±4.32)points,(98.42±5.11)points)were significantly higher than those in the control group((71.59±5.28)points,(90.08±6.03)points)(t=8.289,7.078,both P<0.05).The adverse pregnancy outcomes in the observation group(6.67%(3/45))were lower than that in the control group(22.22%(10/45))(χ2=4.406,P<0.05).Conclusion CGMS combined with CSII has a better regulatory effect on the levels of blood glucose and glucolipid metabolism of pregnant women of advanced maternal age with GDM,and can effectively reduce the risk of adverse pregnancy outcomes.
