Clinical value of semaglutide combined with cardiac rehabilitation training in type 2 diabetes mellitus complicated with coronary heart disease

Abstract：Objective To explore the clinical value of semaglutide combined with cardiac rehabilitation training in patients with type 2 diabetes mellitus(T2DM)complicated with coronary heart disease(CHD).Methods A total of 80 patients with T2DM complicated with CHD treated at Shanghai Jiading District Anting Hospital from February 2024 to March 2025 were retrospectively analyzed.Both the control group and the observation group received foundational drug therapy including glucose-lowering treatment and standard secondary prevention for CHD,along with standardized cardiac rehabilitation training.The observation group additionally received semaglutide therapy.Both groups were treated for 3 months.The clinical efficacy,blood glucose,blood lipid and cardiac function indicators before treatment and 3 months after treatment were compared between two groups,and adverse reactions(nausea,vomiting,diarrhea,constipation and hypoglycemia)of the patients were observed and compared.Results The total effective rate in the observation group(92.86%(39/42))was higher than that in the control group(71.05%(27/38))(χ2=6.570,P=0.010).After treatment,the fasting blood glucose,glycated hemoglobin,2 h postprandial blood glucose,total cholesterol,triglycerides and low density lipoprotein cholesterol levels in the observation group((6.05±1.74)mmol/L,(7.02±1.65)%,(11.05±3.03)mmol/L,(4.34±1.10)mmol/L,(1.49±0.30)mmol/L,(2.26±0.65)mmol/L,respectively)were lower than those in the control group((7.79±2.25)mmol/L,(7.97±2.21)%,(12.41±2.35)mmol/L,(5.02±1.45)mmol/L,(1.98±0.43)mmol/L,(3.28±1.06)mmol/L,respectively)(t=3.889,2.192,2.226,2.377,4.124,5.243,all P<0.05),while the level of high density lipo-protein cholesterol in the observation group((1.62±0.39)mmol/L)was higher than that in the control group((1.30±0.37)mmol/L)(t=3.755,P<0.05).After treatment,the left ventricular end-diastolic diameter and left ventricular end-systolic diameter in the observation group((38.49±4.23)mm and(30.02±1.85)mm,respectively)were lower than those in the control group((41.10±4.52)mm and(32.10±2.88)mm,respectively),and the left ventricular ejection fraction in the observation group((58.12±5.23)%)was higher than that in the control group((53.46±4.27)%)(t=2.668,3.880,4.338,all P<0.05).The difference between the two groups of adverse reactions was not statistically significant(21.43%(9/42)vs 15.79%(6/38))(χ2=0.416,P=0.519).Conclusion The combination of semaglutide and cardiac rehabilitation training in patients with T2DM combined with CHD can improve blood glucose and cardiac function,indicating that the combination may have a good clinical application prospect.
