Rationality analysis of antimicrobial drug use in hospitalized patients with multidrug-resistant organisms infections

Abstract:Objective To investigate the utilization and rationality of antimicrobial agents among 188 hospitalized patients with multidrug-resistant organisms(MDROs)infections.Methods A total of 188 patients with MDRO infections who were hospitalized for treatment in the Xingtai People's Hospital from January 2023 to January 2025 were enrolled in the study.Pathogenic bacteria were identified,classified and subjected to drug susceptibility testing.The utilization of antimicrobial agents was statistically analyzed.Results A total of 205 MDRO strains were detected in the 188 patients.Escherichia coli(68 strains,33.17%)was the predominant species of MDROs.The MDROs were mainly isolated from sputum samples(38.05%),and the patients were predominantly distributed in the Gastrointestinal Surgery Department(18.62%).In terms of antimicrobial agent administration,the utilization rate of cephalosporins was the highest,with 125 cases(27.17%of patients in total);the patients with the antimicrobial agents treatment course ranging between 8 and 14 days were dominant,accounting for 45.74%(86 cases);irrational use of antimicrobial agents leading to MDRO infections was identified in 48 patients(25.53%),including inappropriate dosage in 20 cases(41.67%),improper route of administration in 9 cases(18.75%),irrational combination therapy in 2 cases(4.17%),and inappropriate duration of therapy in 17 cases(35.42%).Effective treatment was achieved in 140 patients(74.47%),while treatment failure occurred in 48 patients(25.53%),with 12 patients(6.38%)suffering from worsening condition.Conclusion MDRO infections predominantly occur in surgical departments and are mostly caused by the gram-negative bacteria,and the isolation rate is the highest in sputum specimens.Cephalosporins are the most frequently used antimicrobial agents during treatment,with the treatment course predominantly ranging from 8 to 14 days,and the rate of irrational drug administration is high.Therefore,efforts should be made from multiple aspects to effectively enhance the rationality of antimicrobial agent usage.
