Efficacy of urokinase and atorvastatin combined with Buyang Huanwu decoction and Shexiang Baoxin pills in the treatment of patients with acute myocardial infarction

Abstract：Objective To observe the effects of urokinase and atorvastatin combined with Buyang Huanwu decoction and Shex-iang Baoxin pills in the treatment of patients with acute myocardial infarction(AMI)and to explore the impact on the levels of serum microRNA(miR)-221 and miR-126.Methods A total of 142 patients with AMI who were treated in the Department of Cardiolo-gy at The People's Hospital of Langfang City from September 2022 to August 2024 were retrospectively enrolled in the study,and 32 cases were discarded by the inclusion and discard criteria,resulting in a total of 110 cases as the study subjects.The cases were assigned to two batches depending on the sequence of recruitment:the baseline group(53 cases)and the study group(57 cases).The baseline group was given urokinase and atorvastatin,and the study group was treated with urokinase and atorvastatin combined with Buyang Huanwu decoction and Shexiang Baoxin pills.The Traditional Chinese Medicine(TCM)symbol scores and cardiac func-tionality indexes(left atrium diameter(LAD),left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF))were observed and compared between the two groups before and after the treatment.The levels of miR-221 and miR-126 were compared between the two groups before and after the treatment.The curative ef-fect and the incidence rates of adverse reactions were compared between the two groups.Results The TCM syndrome scores(chest pressure,breathlessness,heart pounding,giddiness,exhaustion,etc.)of the study group were(0.72±0.24)points,(0.42±0.13)points,(0.62±0.21)points,(0.61±0.19)points and(0.65±0.23)points,respectively,after the treatment for 14 days,lower than(1.31±0.49)points,(0.71±0.24)points,(1.31±0.47)points,(1.34±0.42)points and(1.40±0.48)points of the baseline group(t=8.282,8.152,10.103,12.039,10.751,all P<0.05).The LAD,LVESD,LVEDD and miR-221 levels of the study group were(35.90±2.51)mm,(38.76±2.66)mm,(55.32±5.16)mm and(0.52±0.11),respectively,after the treatment for 14 days,lower than(39.23±2.48)mm,(42.83±3.18)mm,(59.08±5.09)mm and(0.76±0.13)of the baseline group(t=7.060,7.334,3.868,10.417,all P<0.05).The values of LVEF and miR-126 of the study group were(59.94±5.62)%and(1.01±0.12),respectively,higher than(50.26±4.30)%and(0.83±0.07)of the baseline group(t=10.090,9.825,both P<0.05).The total effective rate of the study group was 91.23%(52/57)after the treatment for 14 days,higher than 75.47%(40/53)of the baseline group(χ2=4.982,P<0.05).There was no significant difference in the incidences of adverse reactions between the two groups(10.53%(6/57)vs 22.64%(12/53),χ2=2.945,P>0.05).Conclusion Urokinase and ator-vastatin combined with Buyang Huanwu decoction and Shexiang Baoxin pills can effectively improve TCM syndrome scores of the patient with AMI,reduce the level of miR-221,elevate the level of miR-126,and improve the clinical therapeutic effect.
