Effects of Yishen Congming Decoction combined with donepezil on therapeutic efficacy and blood flow parameters of patients with vascular cognitive impairment

Abstract：Objective To explore the efficacy of Yishen Congming Decoction combined with donepezil in the treatment of patients with vascular cognitive impairment(VCI)and its influence on angiogenesis indicators and cerebral hemodynamics.Methods A double-blind,randomized,placebo-controlled study was conducted.A total of 108 patients with VCI admitted to the Internal Medicine Ward of Shanghai Jiading Hospital of Traditional Chinese Medicine from November 2021 to November 2023 were selected as the research subjects and ran-domly divided into the control group(n=55)and the combination group(n=53)by random number table method.The control group was given Yishen Congming Decoction placebo and donepezil,while the combination group received Yishen Congming Decoction granules and donepezil,and both groups were treated for 90 days.Before treatment,at 30 days of treatment,and at 90 days of treatment,National Institutes of Health Stroke Scale Score(NIHSS),Barthel index,Montreal Cognitive Assessment Scale(MoCA),Mini-Mental State Examination Sale(MMSE)were used to evaluate the degree of eurological deficit,daily living function status,and cognitive function status of the two groups of patients respectively.The cerebral hemodynamic parameters(mean blood flow velocity(Vm),pulsatility index(PI)),angiogenic markers(vas-cular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)),and the incidence of adverse reactions of the two groups of patients were detected.Results At 90 days of treatment,the NIHSS scores and PI levels of the two groups decreased compared with those before treatment(all P<0.05),while the Barthel index,MoCA score and MMSE score,the level of Vm,VEGF and bFGF increased compared with those before treatment(all P<0.05).At 30 days and 90 days of treatment,the NIHSS scores of the combination group((3.67±2.05)points and(2.61±0.56)points)were lower than those of the control group((4.16±1.37)points and(3.55±0.79)points),and the PI levels in the combination group((0.82±0.14)and(0.70±0.13))were also lower than those in the control group((0.88±0.13),(0.81±0.11))(t=2.274,5.379,1.760,4.523,all P<0.05).At 30 and 90 days of treatment,the Barthel index((76.04±2.47)points and(86.25±5.18)points),MoCA scores((23.26±2.37)points and(25.62±2.43)points),MMSE scores((23.91±2.03)points and(26.84±3.06)points),Vm levels((58.63±5.20)cm·s-1 and(63.17±3.72)cm·s-1),VEGF levels((241.81±32.61)pg·mL-1 and(371.76±49.95)pg·mL-1),and bFGF levels((18.72±4.15)pg·mL-1 and(22.86±5.03)pg·mL-1)in the combination group were both higher than those in the control group((68.23±2.15)points,(78.31±6.09)points,(22.08±2.59)points,(22.19±1.95)points,(20.48±2.70)points,(23.75±2.05)points,(55.92±5.21)cm·s-1,(59.17±3.05)cm·s-1,(189.82±32.44)pg·mL-1,(302.63±60.99)pg·mL-1,(15.63±3.87)pg·mL-1,(17.92±4.26)pg·mL-1,respectively)(t=-5.722,-10.025,-1.693,-5.794,-2.635,-3.105,-4.390,-4.104,-8.327,-6.277,3.912,5.237,all P<0.05).The incidence of adverse reactions in the combined group and the control group were 3.77%(2/53)and 20.00%(11/55)respectively(χ2=5.267,P=0.022).Conclusion Yishen Congming Decoction combined with donepezil can effectively improve neu-rological deficits,cognitive function,quality of life,angiogenesis markers,and cerebral hemodynamics in patients with VCI.
