Effect of remimazolam combined with sufentanil anesthesia on elderly patients undergoing laparoscopic total hysterectomy

Abstract:Objective To investigate the effects of remimazolam combined with sufentanil anesthesia on hemodynamics and complications in elderly patients undergoing laparoscopic total hysterectomy.Methods A total of 78 elderly patients who un-derwent laparoscopic total hysterectomy at Huai'an People's Hospital of Hongze District and Huai'an Second People's Hospital from January 2023 to May 2025 were enrolled and randomly divided into an observation group(n=39,remimazolam combined with sufentanil anesthesia)and a control group(n=39,propofol combined with sufentanil anesthesia)using a random number table method.Heart rate(HR)and mean arterial pressure(MAP)at the following time points:5 min after quiet room entry(T1),at tracheal intubation(T2),5 min after pneumoperitoneum establishment(T3),and 5 min after pneumoperitoneum release(T4),at tracheal extu-bation(T5)were compared between the observation group and the control group.Respiratory depression,hypotension,hypertension,nausea and vomiting,and delirium and other complications in patients of the observation group and the control group were record-ed and compared.Results There was no statistically significant difference in HR between the observation group and the control group at T1(P>0.05).The HR values at T2,T3,T4,and T5 were significantly higher than that at T1(all P<0.05),and the HR values in the observation group at T2,T3,T4 and T5((76.23±11.46)times·min-1,(72.61±9.11)times·min-1,(75.31±11.27)times·min-1,(76.43±11.24)times·min-1,respectively)were significantly lower than those in the control group((84.26±11.58)times·min-1,(77.64±10.22)times·min-1,(83.44±8.66)times·min-1,(83.39±12.11)times·min-1,respectively)(t=3.078,2.294,3.572,2.631,all P<0.05).There were no statistically significant differences in MAP at T1 and T3 between the observation group and the control group(both P>0.05),and MAP values at T2,T4 and T5 were significantly higher than that at T1(all P<0.05).The MAP values in the observation group at T2,T4 and T5((87.47±11.12)mmHg(1 mmHg≈0.133 kPa),(91.49±11.45)mmHg,(78.19±9.17)mmHg,respectively)were significantly lower than those in the control group((95.68±12.73)mmHg,(97.35±12.49)mmHg,(83.28±11.27)mmHg,respectively)(t=3.033,2.160,2.188,all P<0.05).The incidence of complications in the observation group was 12.82%(5/39),lower than that in the control group(33.33%(13/39))(χ2=4.622,P<0.05).Conclusion Remimazolam combined with sufentanil anesthesia provides more stable hemodynamics and higher safety in elderly patients undergoing laparoscopic total hysterectomy.
