Efficacy of nifedipine combined with empagliflozin in elderly patients with type 2 diabetes mellitus complicated with hypertension

Abstract:Objective To investigate the efficacy of nifedipine controlled-release tablets combined with empagliflozin in elderly patients with type 2 diabetes mellitus(T2DM)and hypertension.Methods A total of 128 patients with T2DM and hypertension admitted from October 2021 to September 2024 were selected and randomly divided into a study group(64 cases)and a control group(64 cases).During the study,no cases were excluded or dropped from the study group and the control group.The control group received the conventional treatment plan plus nifedipine controlled-release tablets,while the study group received the conventional treatment plan plus nifedipine controlled-release tablets plus empagliflozin.The changes in blood glucose indicators,blood pressure variability,left ventricular mass index and adverse reactions in both groups before and after treatment were recorded and analyzed.Results After treatment,the fasting plasma glucose,2-hour postprandial glucose,and glycosylated hemoglobin levels in the study group and the control group((5.74±1.08)mmol L-1,(7.34±1.45)mmol L-1,(6.34±1.26)mmol L-1,(6.49±1.23)mmol L-1,(8.76±1.98)mmol L-1,(7.65±1.52)mmol L-1,respectively)were lower than those before treatment((8.03±2.17)mmol L-1,(13.16±2.24)mmol L-1,(9.17±2.09)mmol L-1,(8.12±2.30)mmol L-1,(12.87±2.45)mmol L-1,(8.85±1.98)mmol L-1,respectively),with the study group showing lower levels(t=3.666,4.629,5.308,all P<0.05).After treatment,the 24-hour average systolic blood pressure,24-hour systolic blood pressure standard deviation,24-hour average diastolic blood pressure,and 24-hour diastolic blood pressure standard deviation in the study group and the control group((124.19±7.42)mmHg(1 mmHg≈0.133 kPa),(9.28±2.26)mmHg,(78.21±8.37)mmHg,(7.56±1.87)mmHg,(127.56±9.37)mmHg,(11.13±2.79)mmHg,(81.64±9.42)mmHg,(9.14±2.11)mmHg,respectively)were lower than those before treatment((135.24±10.25)mmHg,(15.23±3.64)mmHg,(83.65±9.27)mmHg,(12.84±3.36)mmHg,(133.71±10.62)mmHg,(15.65±3.26)mmHg,(84.32±9.58)mmHg,(13.15±3.18)mmHg,respectively),with the study group showing lower levels(t=2.256,4.122,2.178,4.483,all P<0.05).After treatment,the left ventricular internal diameter,inter-ventricular septum thickness,left ventricular posterior wall thickness,and left ventricular mass index(LVMI)in the study group and the control group((4.35±0.29)cm,(0.92±0.13)cm,(0.88±0.12)cm,(80.72±10.53)g·(m2)-1,(4.48±0.3)cm,(0.99±0.11)cm,(0.94±0.16)cm,(86.45±13.07)g·(m2)-1,respectively)were lower than those before treatment((4.56±0.34)cm,(1.04±0.15)cm,(0.97±0.18)cm,(93.24±18.32)g·(m2)-1,(4.61±0.32)cm,(1.07±0.18)cm,(1.01±0.20)cm,(95.87±17.61)g·(m2)-1,respectively),with the study group showing lower levels than the control group(t=2.450,3.288,2.400,4.731,all P<0.05).The total incidence of ad-verse reactions in the study group was 15.63%(10/64),and that in the control group was 14.06(9/64),with no statistically significant difference between the study group and the control group(χ2=0.062,P>0.05).Conclusion Nifedipine controlled-release tablets combined with empagliflozin are effective in the treatment of elderly patients with T2DM and hypertension.It improves the blood glucose levels and blood pressure variability,reduces LVMI,and has better safety.
