Effect of dronedarone combined with metoprolol on prognosis of patients with persistent atrial fibrillation after radiofrequency catheter ablation

Abstract:Objective To explore the effect of dronedarone combined with metoprolol on postoperative prognosis in patients with persistent atrial fibrillation after radiofrequency catheter ablation(RFCA).Methods A total of 92 patients with persistent atrial fibrillation undergoing RFCA in the First Affiliated Hospital of Xinxiang Medical University from January 2022 to October 2023 were selected as the study subjects.They were randomly divided into a single group and a combination group using a random number table,with 46 patients in each group.The patients in the single group received oral metoprolol succinate sustained-release tablets for postoperative antiarrhythmic treatment in RFCA,while those in the combination group received dronedarone tablets for sinus rhythm maintenance on this basis.The serum inflammatory cytokine indicators,myocardial injury markers,cardiac function indicators,early recurrence,quality of life,and adverse reactions were compared between the two groups before surgery,1 month after surgery,and 3 months after surgery.Results The levels of C-reactive protein(CRP),interleukin 6(IL-6)and tumor necro-sis factor α(TNF-α)in the two groups at 1 and 3 months after surgery were lower than those before treatment(all P<0.05),and the levels of CRP,IL-6 and TNF-α in the combined group were lower than those in the single group(t1 month after surgery=3.565,2.356,2.192,t3 months after surgery=3.189,2.168,2.309,all P<0.05).The levels of soluble matrix lysin 2(sST2),galectin-3 and N-terminal B-type natriuretic peptide(NT-proBNP)in the two groups at 1 and 3 months after surgery were lower than those before treatment(all P<0.05),and the levels of sST2,galectin-3 and NT-proBNP in the combined group were lower than those in the single group(t1 month after surgery=6.026,2.690,2.283,t3 month after surgery=8.286,2.342,5.095,all P<0.05).At 1 and 3 months after surgery,the LAD and LVEF of the two groups were lower than those before treatment,and the left ventricular ejection fraction(LVEF)was higher than that before treatment(P<0.05),and the left atrial diameter(LAD)of the combined group was lower than that of the single group,and the LVEF was higher than that of the single group(t1 month after surgery=2.027,2.182,t3 month after surgery=2.045,2.257,all P<0.05).The scores of physical health and mental health in the two groups at 1 and 3 months after surgery were higher than those before treatment(all P<0.05),and the scores of physical health and mental health in the combined group were higher than those in the single group(t1 month after surgery=2.072,2.577,t3 month after surgery=2.142,2.000,all P<0.05).The early postoperative recurrence rate of persistent atrial fibrillation in the combined group after RFCA was 8.70%,which was obviously lower than the 23.91%in the single group(χ2=3.903,P=0.048).There was no signifi-cant difference in the overall incidence of adverse reactions between the two groups(χ2=0.472,P=0.492).Conclusion The combi-nation of dronedarone and metoprolol can obviously reduce the level of inflammation in patients with persistent atrial fibrillation after RFCA surgery,alleviate the myocardial damage caused by surgery,improve cardiac function,and reduce the early recurrence rate and improve the quality of life of postoperative patients.It has certain promotion and application value.
