Efficacy of alirocumab combined with ticagrelor in the treatment of patients with acute ST-segment elevation myocardial infarction

Abstract:Objective To evaluate the clinical efficacy of alirocumab combined with ticagrelor in patients with acute ST-seg-ment elevation myocardial infarction.Methods A total of 200 patients with acute ST-segment elevation myocardial infarction admitted to 3201 Hospital from May 2022 to December 2024 were divided into the combination group and the ticagrelor group based on different treatment regimens,with 100 cases in each group.All patients underwent percutaneous coronary intervention after admission.The ticagrelor group received ticagrelor treatment after intervention therapy,while the combination group received additional alirocumab treatment on the basis of the ticagrelor group's medication,with a treatment course of 6 months.The clinical efficacy,blood lipid levels,vascular recanalization rate,inflammation levels,cardiac function conditions and the probability of ad-verse reactions were observed and compared between the ticagrelor group and the combination group.Results The total effective rate of the combination group was 95.00%(95/100),which was higher than that of the ticagrelor group at 82.00%(82/100)(χ2=8.303,P<0.05).The vascular recanalization rate in the combination group was 91.00%(91/100),which was higher than that in the ticagrelor group at 78.00%(78/100)(χ2=6.452,P<0.05).After treatment,the level of total cholesterol,low-density lipoprotein cholesterol and triglycerides in the combination group((3.16±0.50)mmol·L-1,(1.43±0.40)mmol·L-1,(1.98±0.46)mmol·L-1,respectively)were all lower than those in the ticagrelor group((3.64±0.51)mmol·L-1,(1.59±0.42)mmol·L-1,(2.13±0.41)mmol·L-1,respectively)(t=6.721,2.759,2.434,all P<0.05).After treatment,the left ventricular ejection fraction and the maximum blood flow velocity of left ventricle in the early diastolic/late diastolic in the combination group((62.05±5.21)%,(1.47±0.27),respectively)were both higher than those in the ticagrelor group((57.13±5.14)%,(1.24±0.15),respectively)(t=6.722,7.447,both P<0.05),and the inner diameter at the end of left ventricular contraction((34.01±3.08)mm)was lower than that in the ticagrelor group((36.99±3.89)mm)(t=6.006,P<0.05);the levels of the high-sensitivity C-reactive protein and interleukin-6 in the combination group((1.51±0.31)mg·L-1,(13.62±2.59)ng·L-1,respectively)were both lower than those in the ticagrelor group((1.62±0.32)mg·L-1,(16.41±3.60)ng·L-1,respectively)(t=2.469,6.291,both P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the ticagrelor group and the combination group(13.00%(13/100)vs 11.00%(11/100))(χ2=0.189,P>0.05).Conclusion The combined use of alirocumab and ticagrelor can effectively improve the clinical efficacy of patients with acute ST-segment elevation myocardial infarction,regulate blood lipid and inflammation levels,improve myocardial function,promote vascular recanalization,and has good safety.
