Efficacy of ulinastatin combined with sustained low-efficiency dialysis in patients with sepsis-associated acute kidney injury

Abstract:Objective To observe the clinical efficacy of ulinastatin combined with sustained low-efficiency dialy-sis(SLED)in treating sepsis-associated acute kidney injury(SA-AKI).Methods A total of 102 patients with SA-AKI who were admitted to Department of Critical Care Medicine,the First Affiliated Hospital of Xinjiang Medical University from June 2023 to February 2025 were selected as the study objects and randomly divided into a combination group and a con-trol group according to random number table method,with 51 cases in each group.The control group received convention-al therapy plus sustained low-efficiency dialysis(SLED),whereas the combination group received conventional therapy plus SLED plus ulinastatin.The levels of serum creatinine,uric acid,urea nitrogen,kidney injury molecule-1,procalcitonin,inter-leukin-6,24-hour urine volume,and acute physiology and chronic health evaluationⅡ(APACHEⅡ)score of the two groups before treatment and 7 days after treatment were compared.Results After treatment,the combination group had signifi-cantly lower levels of uric acid,serum creatinine,urea nitrogen,kidney injury molecule-1,procalcitonin,interleukin-6,and APACHEⅡscore((293.35±57.26)μmol·L-1,(148.25±45.61)μmol·L-1,(12.41±2.46)mmol·L-1,(12.34±2.65)μg·L-1,(2.85±0.76)ng·mL-1,(32.53±4.28)pg·mL-1,(11.59±2.18)points)than those of the control group((346.82±61.83)μmol·L-1,(183.70±51.23)μmol·L-1,(16.82±2.59)mmol·L-1,(15.74±3.06)μg·L-1,(5.02±0.84)ng·mL-1,(45.63±5.15)pg·mL-1,(15.86±3.05)points).The differences were all statistically significant(t=4.996,4.070,9.721,6.614,15.084,15.404,8.968,all P<0.05).The 24-hour urine volume of the combination group((1 006.35±124.78)mL)was higher than that of the control group((759.64±53.92)mL)(t=14.291,P<0.05).Conclusion Ulinastatin combined with SLED can effectively improve the renal function of patients with SA-AKI,reduce kidney damage and inflammatory response,and better control the patients'condition.
