Comparison of efficacy and cost of different courses of antibiotics after artificial femoral head replacement

Abstract:Objective To investigate the clinical efficacy and cost of short-course and conventional-course antibiot-ic therapy after artificial femoral head replacement(AFHR).Methods A total of 100 patients who underwent AFHR at Wuxi Traditional Chinese Medicine Hospital between January 2019 and January 2024 were included and divided into the short-course group(n=50)and the conventional-course group(n=50)using a random number table.The short-course group received short course of antibiotic treatment for infection prevention,while the conventional-course group received con-ventional course of antibiotic treatment.The incision healing,infection of various systems,inflammatory factors,operation time,supplemental use of antibiotic and postoperative treatment situation were compared between the short-course group and the conventional-course group.Results The Grade A postoperative incision healing rate was 94.0%(47/50)for the short-course group and 96.0%(48/50)for the conventional-course group,while the postoperative incision infection rate was 0%and 2.0%(1/50),respectively.There were no statistically significant differences between the two groups for these outcomes(χ²=0.000,P=0.998;χ²=0.000,P=0.999).At 24 hours post-surgery,white blood cell count((9.73±3.02)×109·L-1 vs(9.14±2.75)×109·L-1),neutrophil count((8.18±2.84)×109·L-1 vs(7.44±2.54)×109·L-1),C-reactive protein((66.65±44.93)mg·L-1 vs(73.33±58.49)mg·L-1),procalcitonin((0.43±0.13)ng·mL-1 vs(0.43±0.13)ng·mL-1),and erythrocyte sedimentation rate((44.66±13.31)mm·h-1 vs(40.94±10.99)mm·h-1 were all significantly higher than preoperative levels in both groups(all P<0.05).There were no additional antibiotics administered during surgery in either group.The mean operation time for the short-course group was(65.8±9.37)min,which was not significantly different from the conventional-course group's(66.4±10.1)min(t=0.264,P=0.793).Patients in the short-course group had a significantly shorter postoperative hospital stay of(13.02±3.21)days compared to(15.22±4.03)days in the conventional-course group(t=3.019,P=0.003),and their antibiotic costs were significantly lower at(429.42±293.84)yuan compared to(670.71±384.65)yuan(t=3.486,P<0.001).No significant differences were found between the two groups for body temperature on postoperative days 1,2,and 3,nor for WBC count and C-reactive protein on postoperative days 1 and 3(all P>0.05).Conclusion Short-course antibiotic treatment for AFHR patients can reduce hospitalization costs and postoperative hos-pital stays while achieving the same clinical efficacy as conventional-course antibiotic treatment.
