Clinical evaluation of Qianyang Yuyin granules combined with valsartan and amlodipine on treatment of elderly patients with hypertension

Abstract：Objective To explore the efficacy of Qianyang Yuyin granules combined with valsartan and amlodipine in treat-ment of elderly patients with hypertension and observe the influence on blood pressure variability and inflammatory responses.Methods A total of 75 elderly patients with hypertension who were treated with valsartan combined with amlodipine from June 2023 to June 2024 in Jiangsu Provincial Hospital of Traditional Chinese Medicine were assigned as the group A,and 75 elderly patients with hyper-tension who received Qianyang Yuyin granules on this basis were assigned as the group B.The course of treatment was 8 weeks,and the follow-up period was 10 weeks.TCM syndromes scores,blood pressure and blood pressure variability indicators(24 h systolic blood pressure variability(SBPV),24 h diastolic blood pressure variability(DBPV),daytime DBPV,nighttime DBPV,daytime SBPV,nighttime SBPV),in-flammatory factors(interleukin-1β(IL-1β),C-reactive protein(CRP)),serum angiotensin Ⅱ(Ang Ⅱ)and aldosterone(ALD)were observed and compared between the two groups before and after the treatment.Results The total scores of TCM syndromes of the group B and group A were respectively(11.88±2.96)points and(15.19±2.49)points after the treatment,lower than(23.48±1.61)points and(23.40±1.72)points before the treatment,and the total score of the group B was lower than that of the group A(t=7.411,P<0.05).The systolic blood pressure and diastolic blood pressure were respectively(146.48±9.62)mm Hg(1 mm Hg≈0.133 kPa)and(90.11±8.31)mm Hg,(137.25±9.17)mm Hg and(82.56±8.60)mm Hg in the group B after the treatment 8 weeks and 10 weeks of follow-up,and these indicators were(150.83±9.27)mm Hg and(93.44±8.79)mm Hg,(140.79±8.95)mm Hg and(85.78±8.41)mm Hg in the group A,lower than((173.69±9.56)mm Hg,(102.07±8.87)mm Hg;(173.15±9.88)mm Hg,(101.77±9.03)mm Hg)before the treatment;the two indexes of the group B were lower than those of the group A(t=2.820、2.393、2.384、2.318,all P<0.05).The DBPV,SBPV,daytime DBPV,nighttime DBPV,daytime SBPV and nighttime SBPV of the group B and group A were respectively(11.73±1.76)mm Hg,(10.28±1.38)mm Hg,(9.94±2.21)mm Hg,(8.11±1.63)mm Hg,(9.87±1.77)mm Hg,(8.92±1.65)mm Hg;(12.38±1.53)mm Hg,(10.94±1.59)mm Hg,(10.77±2.13)mm Hg,(8.74±1.79)mm Hg,(10.69±2.05)mm Hg and(9.64±2.07)mm Hg after the treatment,lower than(15.33±2.15)mm Hg,(14.78±1.82)mm Hg,(15.41±2.49)mm Hg,(10.74±2.48)mm Hg,(13.05±2.63)mm Hg,(14.44±2.74)mm Hg;(14.89±2.34)mm Hg,(14.69±1.64)mm Hg,(15.28±2.16)mm Hg,(11.15±2.56)mm Hg,(12.76±2.26)mm Hg and(14.73±2.61)mm Hg before the treatment;the above indexes of the group B were lower than those of the group A(t=2.414、2.715、2.342、2.254、2.622、2.356,all P<0.05).The levels of IL-1β and CRP were respectively(15.27±2.36)pg·mL﹣1 and(4.86±1.22)mg·L-1 in the group B after the treatment,(16.14±2.42)pg·mL﹣1 and(5.37±1.36)mg·L-1 in the group A,lower than(30.62±3.11)pg·mL﹣1,(8.18±1.74)mg·L-1;(31.16±3.24)pg·mL﹣1 and(8.05±1.63)mg·L-1 before the treatment;the levels of the above indexes of the group B were lower than those of the group A(t=2.229、2.417,both P<0.05).The levels of serum Ang Ⅱ and ALD of the group B and group A were respectively(0.56±0.14)pg·mL﹣1,(0.08±0.02)ng·mL﹣1;(0.63±0.13)pg·mL﹣1 and(0.09±0.03)ng·mL﹣1 after the treatment,lower than(0.72±0.11)pg·mL﹣1,(0.12±0.04)ng·mL﹣1;(0.71±0.12)pg·mL﹣1 and(0.11±0.04)ng·mL﹣1 before the treatment;the levels of the above indexes of the group B were lower than those of the group A(t=3.173、2.402,both P<0.05).The total incidence of adverse reactions in group A and group B was 5.33％and 9.33％,respectively,with no statistically significant difference(P>0.05).Conclusion Qianyang Yuyin granules combined with valsartan and amlodipine has a good effect on the treatment of elderly hy-pertension.It can effectively control the blood pressure and improve the blood pressure variability,and help to inhibit the inflamma-tory response and regulate the expressions of vasoactive substances.
