Efficacy of dydrogesterone combined with phloroglucinol in fetal retention and its impact on sex hormones changes in pregnant women with threatened miscarriage

Abstract:Objective To observe the abortion prevention effect of combined therapy with dydrogesterone and phloroglucinol on pregnant women with threatened abortion and its influence on levels of serum sex hormones.Methods In this study,154 patients with threatened abortion admitted to Wuhan Children's Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology from February 2022 to January 2023 were selected as the research subjects and divided into the observation group and the con-trol group by coin tossing method,with 77 patients in each group.The patients in the control group were treated with dydrogesterone tablets,while those in the observation group were given dydrogesterone tablets combined with intravenous infusion of phloroglucinol.All the patients in both groups were treated for 10 days.The clinical efficacy and abortion prevention effect were compared.The clinical indicators(vaginal bleeding stopping,symptom disappearance,uterine contraction disappearance)and changes of serum sex hormones(serum progesterone(P),estradiol(E2),β-human chorionic gonadotropin(β-HCG)),coagulation function indicators(fibrinogen(FIB),platelet count(PLT),D-di-mer(D-D))before and after treatment were observed and counted in both groups,and the adverse events during treatment were recorded.Results The overall effective rate and success rate of abortion prevention with 94.81%and 89.61%in the observation group were higher than 81.82%and 66.23%in the control group,and the differences were statistically significant(χ2=6.291,12.229,both P<0.05).The vaginal bleeding stopping time,uterine contraction disappearance time,time of symptom disappearance in the observation group were(2.01±0.33)d,(3.01±0.45)d and(6.10±1.56)d,which were shorter than those of(2.63±0.27)d,(3.28±0.55)d and(7.59±2.18)d in the control group,and the differences were statistically significant(t=11.871,3.148,4.627,all P<0.05).After 10 days of treatment,the levels of P,E2 and β-HCG in the observation group were(78.59±4.17)ng·mL-1,(615.36±16.57)pg·mL-1 and(55.45±5.51)IU·mL-1 and they were higher than those of(63.89±5.76)ng·mL-1,(588.53±19.36)pg·mL-1 and(50.26±4.64)IU·mL-1 in the control group,and the differences were statistically signif-icant(t=18.140,9.239,6.322,all P<0.05),while the FIB level,PLT level and D-D level of(2.56±1.27)g·L-1,(179.67±48.22)×109·L-1 and(1.46±0.37)μg·L-1 were significantly lower than those of(3.35±1.78)g·L-1,(198.68±51.26)×109·L-1 and(1.69±0.57)μg·L-1 in the control group,and the differences were statistically significant(t=3.170,2.370,2.970,all P<0.05).There were no statistical differences in the incidence rates of adverse drug reactions and adverse pregnancy outcomes between the observation group(11.69%and 13.05%)and the control group(10.39%and 21.56%),and the differences were not statistically significant(χ2=0.066,1.535,both P>0.05).Conclusion The combination of dydrogesterone and phloroglucinol has exact efficacy in the treatment of threatened abortion.Compared with dydrogesterone table regimen,it can more significantly enhance the abortion prevention effect,accelerate the symp-tom relief,and correct the abnormal levels of sex hormones and coagulation function,and has higher drug safety.
