Efficacy of edaravone dextrocamphenol in treatment of elderly patients with acute cerebral infarction and impact on vascular endothelial function

Abstract：Objective To explore the efficacy of edaravone dextrocamphenol in treatment of elderly patients with acute cerebral infarction and observe the impact on vascular endothelial function.Methods A total of 118 elderly patients with acute cerebral in-farction who were admitted to the Zhengzhou Central Hospital from December 2023 to November 2024 were randomly divided into the control group(n=59)and the combination group(n=59).Both groups were treated with intravenous thrombolysis with alteplase,while the combination group was given additional treatment with edaravone dextrocamphenol.The two groups were treated for 14 days.The clinical curative effect,vascular endothelial function indexes,cerebral hemodynamics indicators,and incidence of adverse reactions were observed and compared between the two groups before and after treatment.Results The total effective rate was 83.05%(49/59)in the control group,lower than 98.31%(58/59)in the combination group,with a statistically significant difference(χ2=8.121,P=0.004).The levels of serum endothelin 1(ET-1)and nitric oxide of both groups declined after treatment,while the average level of serum testosterone(T)was elevated.The serum T level of the combination group was(5.97±1.36)nmol·L-1,higher than(4.12±1.05)nmol·L-1 of the control group,while the levels of ET-1 and nitric oxide of the combination group were respectively(65.02±5.85)ng·L-1 and(31.01±3.10)μmol·L-1,lower than(72.33±5.78)ng·L-1 and(45.03±6.39)μmol·L-1 of the control group,with statistically significant differences(t=8.266,6.858,15.163,P<0.05).After treatment,the levels of mean cerebral blood flow velocity(Vm)and pulsatility index(PI)of both groups were elevated,while the vascular resistance index(RI)level was decreased.The Vm and PI levels of the combination group were respectively(64.14±6.78)cm·s-1 and(23.65±0.95),higher than(53.02±5.61)cm·s-1 and(19.33±0.85)of the control group;the RI level of the combination group was(0.71±0.02),lower than(0.99±0.12)of the control group,with statistically significant differences(t=9.705,26.021,17.695,all P<0.05).The incidence of adverse reactions was 3.39%(2/59)in the control group and 1.69%(1/59)in the combination group,and there was no significant difference between the two groups(χ2<0.001,P=1.000).Conclusion Edaravone dextrocamphenol can achieve remarkable effect on treatment of the elderly patients with acute cerebral infarction,boost the vascular endothelial function and improve the cerebral hemodynamics.
