Clinical evaluation of Xueshuantong combined with betahistine in the treatment of patients with posterior circulation ischemic vertigo

Abstract：Objective To explore the influence of Xueshuantong combined with betahistine on vertebrobasilar hemo-dynamics,peripheral blood calcitonin gene-related peptide(CGRP),endothelial nitric oxide synthase(eNOS)and plasma en-dothelin-1(ET-1)in patients with posterior circulation ischemic vertigo.Methods A total of 86 patients with posterior circulation ischemic vertigo admitted to Handan City Eye Hospital from October 2022 to April 2024 were selected as the re-search subjects.According to the simplified random grouping method(single-double number method),these 86 patients were divided into the control group(single number)and study group(double number),with 43 in each group.After conventional treatment was given,the patients in the control group were treated with betahistine while those in the study group with Xue-shuantong combined with betahistine.The clinical efficacy after treatment and dizziness conditions(Dizziness Assessment Rating Scale(DARS),Dizziness Handicap Inventory(DHI)),vertebrobasilar hemodynamics,hemorheological indicators and CGRP,eNOS and ET-1 before and after treatment were compared between the two groups.The adverse reactions were count-ed in both groups during treatment.Results The total effective rate of treatment was 83.72% (36/43)and 97.67% (42/43)in control group and study group,respectively(χ2=4.961,P=0.026).After treatment,the DARS score and DHI score in the con-trol group and study group were(3.88±0.33)points,(63.42±6.41)points,(2.96±0.42)points and(54.96±5.35)points,which were all lower than those of(5.18±0.46)points,(88.74±5.89)points,(5.22±0.41)points and(88.56±5.82)points before treat-ment,and the scores in the study group were lower than those in the control(t=8.699,6.644,both P<0.05).Basilar artery flow dynamic level,the left and right vertebral artery flow velocities in the control group and study group were(45.57±6.72)cm·s-1,(46.04±5.32)cm·s-1,(45.28±4.98)cm·s-1,(48.61±7.37)cm·s-1,(49.81±5.48)cm·s-1 and(48.62±5.31)cm·s-1 after treatment,which were higher than those of(41.46±5.73)cm·s-1,(41.68±4.39)cm·s-1,(39.41±3.92)cm·s-1,(41.57±5.72)cm·s-1,(41.67±4.42)cm·s-1 and(39.33±4.07)cm·s-1 before treatment,and the above indicators in the study group were higher than those in the control(t=1.999,3.237,3.009,all P<0.05).The plasma viscosity,erythrocyte aggregation index and fibrinogen in the control group and study group after treatment((1.97±0.39)mPa·s,(1.89±0.53),(3.82±0.56)g·L-1,(1.66±0.35)mPa·s,(1.51±0.44),and(3.16±0.42)g·L-1)were lower than those of((2.76±0.43)mPa·s,(3.03±0.94),(4.89±0.82)g·L-1,(2.72±0.41)mPa·s,(3.07±0.92),(4.84±0.87)g·L-1)before treatment,and these indicators were lower in the study group than those in the control(t=3.879,3.617,6.183,all P<0.05).The levels of eNOS and CGRP in the control group and study group after treatment((34.57±4.27)μmol·L-1,(32.88±5.72)ng·L-1,(37.31±5.46)μmol·L-1,(37.22±6.45)ng·L-1)were higher than those of((23.46±3.33)μmol·L-1,(29.32±5.38)ng·L-1,(23.32±3.53)μmol·L-1,(29.67±5.42)ng·L-1)before treatment,and they were improved significantly in the study group as compared with the control(t=2.592,3.301,all P<0.05).The level of ET-1 in the control group and study group after treatment((90.28±9.98)ng·mL-1,(78.92±8.21)ng·mL-1)was lower than that of(109.01±13.92)ng·mL-1,(109.65±13.07)ng·mL-1)before treatment,and it was lower in the study group than in the control(t=5.764,P<0.05).The incidence of adverse reactions was 6.98% (3/43)in the control group and 9.30% (4/43)in the study group(χ2=0.156,P=0.693).Conclusion Xueshuantong combined with betahistine can effectively reduce vertigo symptoms,improve vertebrobasilar hemodynamics and vascular endothelial function,and promote vasodilation in patients with posterior circu-lation ischemic vertigo,and it has good safety.
