Clinical evaluation of budesonide combined with Shufengqingre Zhike prescription on treatment of patients with chronic obstructive pulmonary disease

Abstract：Objective To observe the effect of budesonide combined with Shufengqingre Zhike prescription on treatment of patients with chronic obstructive pulmonary disease(COPD)of phlegm-heat obstructing the lung pattern and the impact on the levels of interleukin-6(IL-6),soluble triggering receptor expressed on myeloid cells(sTREM-1)and PCT.Methods A sin-gle-blind randomized controlled trial was conducted.A total of 106 COPD patients who were admitted to the Shanghai Pudong New Area People's Hospital from June 2021 to June 2024 were enrolled in the study and randomly divided into two groups.52 patients in the control group were administered budesonide monotherapy,whereas 54 patients in the observation group were treated with budesonide in combination with Shufengqingre Zhike prescription.The two groups were treated continuously for 14 days.The clin-ical efficacy,time for disappearance of clinical symptoms,TCM syndrome scores,lung function indexes,and levels of serum IL-6,sTREM-1 and PCT were observed and compared between the two groups.Results The total effective rate of the observation group was 94.44%,higher than 80.77% of the control group(χ2=4.604,P=0.032).The scores of cough,fever,sputum and wheezing of the two groups were respectively(2.13±0.32)points,(3.09±0.45)points,(2.32±0.38)points,(3.17±0.45)points,(2.0 8±0.29)points,(3.14±0.42)points,(2.25±0.29)points and(3.11±0.42)points after the treatment,lower than(4.37±0.63)points,(4.41±0.61)points,(4.71±0.71)points,(4.65±0.67)points,(4.59±0.69)points,(4.56±0.65)points,(4.59±0.65)points and(4.56±0.61)points before the treatment;the above indexes of the observation group were lower than those of the control group(t=12.695,10.522,15.169,12.307,all P<0.05).The pulmonary function indicators of FVC,Ti/Tt and FEV1/FVC of the two groups were respectively(3.38±0.68)L,(2.82±0.47)L,(43.56±6.71)%,(39.87±5.85)%,(78.56±6.61)% and(72.87±5.85)% after the treatment,higher than(2.53±0.43)L,(2.58±0.39)L,(35.23±5.19)%,(35.32±5.24)%,(65.46±7.19)% and(65.58±7.24)% before the treatment;the improvement of lung function of the observation group was better than that of the control group(t=4.914,3.013,4.687,all P<0.05).The pH and PaO2 lev-els of the two groups were higher after the treatment than before the treatment[(7.41±0.26)mm Hg,(7.31±0.24)mm Hg,(83.26±6.48)mm Hg(1 mm Hg≈0.133 kPa),(74.87±5.94)mm Hg];the levels of the above indexes of the observation group were higher than those of the control group(t=2.056,6.941,P<0.05).The PaCO2 levels of the two groups were lower after the treatment[(42.78±3.85)mm Hg,(55.87±4.12)mm Hg]than before the treatment[(66.58±4.75)mm Hg,(66.49±4.69)mm Hg];the levels of the above indexes of the obser-vation group were lower than those of the control group(t=16.908,P<0.05).The levels of IL-6,sTREM-1 and PCT of the two groups were respectively(32.13±4.20)ng·L-1,(43.09±5.54)ng·L-1,(25.86±3.56)ng·L-1,(34.75±5.12)ng·L-1,(0.12±0.02)g·mL-1 and(0.20±0.04)g·mL-1 after the treatment,lower than(86.27±9.85)ng·L-1,(86.59±9.45)ng·L-1,(92.75±8.74)ng·L-1,(92.62±8.92)ng·L-1,(0.38±0.06)g·mL-1 and(0.36±0.07)g·mL-1 before the treatment;the levels of the above indexes of the observation group were lower than those of the control group(t=11.505,10.412,13.097,all P<0.05).Conclusion Budesonide combined with Shufengqingre Zhike prescription can achieve re-markable effect on treatment of the patients with COPD and reduce the levels of serum IL-6,sTREM-1 and PCT.
