Efficacy of budesonide combined with different doses of dexamethasone on treatment of acute infectious laryngitis children complicated with laryngeal obstruction

Abstract：Objective To explore the effects of budesonide combined with different doses of dexamethasone on treatment of the acute infectious laryngitis children complicated with laryngeal obstruction.Methods A total of 150 acute infectious laryngitis chil-dren who were complicated with laryngeal obstruction and treated in the Xingyuan Hospital of Yulin from July 2021 to October 2024 were enrolled in the study and randomly divided into two groups.The control group(n=75)was given budesonide nebulization(1 mg per dose,twice daily,q12 h)combined with a conventional dose of dexamethasone(0.5 mg·kg-1·d-1,single intravenous push);the ob-servation group(n=75)was treated with budesonide nebulization(1 mg per dose,q12 h)combined with a high dose of dexamethasone(1 mg·kg-1·d-1,single intravenous push).Both groups were treated for one week.The clinical efficacy,symptom disappearance time,changes in serum inflammatory factors before and after treatment,and adverse reactions were compared between the two groups.Results The effective rate of treatment of the observation group was higher than that of the control group((93.33%,70/75)vs(80.00%,60/75),P<0.05).Specifically,for the children with grade Ⅲ laryngeal obstruction,the effective rate of treatment of the observation group was(83.33%,5/6),higher than(0,0/5)of the control group(χ2=5.769,4.468,both P<0.05).The subsided duration of symptoms such as hoarseness,dyspnea,barking cough and inspiratory stridor were respectively(2.46±0.34)days,(0.81±0.26)days,(1.67±0.46)days and(2.17±0.54)days in the observation group,shorter than(2.92±0.45)days,(1.19±0.38)days,(2.03±0.49)days and(2.56±0.58)days in the control group(t=7.036,7.147,4.639,4.262,all P<0.05).After treatment,the levels of serum C-reactive protein,tumor necrosis factor-α,and interleukin-6 of both groups were decreased;the levels of the above indexes of the observation group were(6.12±1.59)mg·L-1,(8.49±2.19)ng·L-1 and(36.11±9.62)pg·mL-1,respectively,lower than(10.03±1.82)mg·L-1,(11.17±2.24)ng·L-1 and(48.36±11.25)pg·mL-1 of the control group.(t=14.011,7.409,7.167,all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(6.67% vs 10.67% )(χ2=7.538,P>0.05).Conclusion Budesonide combined with different doses of dexamethasone can achieve certain effect on treatment of the acute infectious laryngitis children complicated with laryngeal obstruction.Among them,a dose of 1.0 mg·kg-1·d-1 of dexamethasone combined with budesonide shows more remarkable anti-inflammatory effect and alleviate the symptoms faster,with its safety similar to conventional dosages.
