Clinical efficacy of ulinastatin combined with somatostatin on treatment of patients with hyperlipidemia-induced acute pancreatitis

Abstract:Objective To explore the effect of the combination of ulinastatin and somatostatin on treatment of patients with hy-perlipidemia-induced acute pancreatitis(HLAP)and observe its impact on inflammatory cytokines.Methods Patients with HLAP diagnosed at Yuncheng Central Hospital Affiliated to Shanxi Medical University from August 2021 to December 2024 were selected and divided into a control group and an observation group according to the order of enrollment.A total of 162 patients completed the observation,including 79 in the control group and 83 in the observation group.The clinical efficacy,levels of interleukin-6,C-reac-tive protein,procalcitonin,pancreatic function and metabolism-related indicators(serum amylase,triglycerides,lipase),and adverse reactions were compared between the two groups.Results The time to relief of abdominal distension,abdominal pain,recovery of bowel sounds,and hospital stay duration were significantly shorter in the observation group((4.31±0.57)days,(3.82±0.49)days,(4.67±0.63)days,(8.36±1.34)days)than in the control group((4.73±0.61)days,(4.21±0.55)days,(5.12±0.72)days,(10.23±1.61)days)(t=4.530,4.770,4.239,8.050,all P<0.01).After the treatment,the levels of serum CRP,IL-6 and PCT decreased in both groups,and the levels of the above indexes were lower in the observation group((36.23±5.61)mg·L-1,(33.41±4.42)pg·mL-1,(0.051±0.009)ng·mL-1)than in the control group((50.47±6.88)mg·L-1,(51.67±5.92)pg·mL-1,(0.072±0.011)ng·mL-1)(t=14.469,22.317,13.327,all P<0.01).Similarly,the levels of serum AMY,TG and LPS decreased in both groups after the treatment,and the levels of the above indexes were lower in the observation group((77.84±8.90)U·L-1,(4.03±1.52)mmol·L-1,(105.42±11.53)U·L-1)than in the control group((121.17±12.79)U·L-1,(6.12±1.09)mmol·L-1,(144.37±15.62)U·L-1)(t=25.129,10.013,18.117,all P<0.01).The incidence of adverse reactions was 6.33% (5/79)in the control group and 7.23% (6/83)in the observation group.There was no significant difference in the incidence of adverse reactions between the two groups(χ2=0.052,P=0.820).Conclusion The com-bination of ulinastatin and somatostatin can achieve remarkable effect on treatment of the patients with HLAP,effectively alleviate symptoms,shorten hospital stay duration,reduce inflammatory cytokine levels and promote pancreatic function recovery,and it has a certain degree of safety.
