Efficacy of bifidobacterium combined with insulin on patients with gestational diabetes mellitu and its impact on perinatal outcomes

Abstract：Objective To evaluate the clinical efficacy of bifidobacterium combined with insulin aspart and insulin detemir in the treatment of patients with gestational diabetes mellitu and its impact on perinatal outcomes.Methods A total of 195 ges-tational diabetes mellitus patients who were admitted to the General Hospital of Northern Theater Command from January 2022 to December 2023 were selected and divided into the control group(n=97)and the combination group(n=98)based on a random num-ber table method.The control group was treated with insulin aspart and insulin detemir,and the combination group was additionally treated with bifidobacterium on this basis.The blood glucose indexes,insulin resistance index,lipid metabolism indexes,inflamma-tory factor levels and adverse pregnancy outcomes before treatment and before delivery were compared between the two groups.Results Before delivery,the combination group had reduced levels of fasting blood glucose((4.53±0.36)mmol·L-1),2-hour post-meal blood glucose((5.75±0.87)mmol·L-1),and glycosylated hemoglobin(5.12±0.51)% )compared to the control group((4.84±0.32)mmol·L-1,(6.43±1.04)mmol·L-1,(5.51±0.63)% )(t=6.353,4.954,4.753,all P<0.05).Before delivery,the levels of fasting in-sulin and insulin resistance index in the combination group((4.72±0.92)μIU·mL-1,(1.48±0.50),respectively)were lower than those in the control group((6.20±1.24)μIU·mL-1,(2.26±0.75),respectively)(t=9.472,8.553,both P<0.05).Before delivery,the levels of total cholesterol,triglyceride and low-density lipoprotein cholesterol in the combination group((3.04±1.02)mmol·L-1,(1.16±0.17)mmol·L-1,(2.52±0.36)mmol·L-1,respectively)were better than those in the control group((3.75±1.10)mmol·L-1,(1.35±0.24)mmol·L-1,(2.91±0.42)mmol·L-1,respectively)(t=4.016,6.384,6.964,all P<0.05).The levels of high-sensitivity C-reactive protein and interleukin-6 in the combination group((1.61±0.52)mg·L-1,(3.08±0.54)pg·mL-1,respectively)were lower than those in the control group((2.23±0.65)mg·L-1,(3.95±0.73)pg·mL-1,respectively)(t=7.359,9.468,both P<0.05).The total incidence of premature birth,pregnancy complications,macrosomia,neonatal asphyxia and neonatal hypoglycemia in the combination group was 17.35% (17/98),which was lower than that in the control group(31.96%,31/97)(χ2=5.609,P<0.05).Conclusion The therapeutic regimen of bifidobacterium combined with insulin aspart and insulin detemir can not only effectively stabilize blood glucose of pa-tients with gestational diabetes mellitus,but also improves insulin resistance and lipid metabolism,lower the levels of inflammatory factors,thereby potentially reducing the risk of adverse pregnancy outcomes.
