Clinical efficacy of bifidobacterium combined with vitamin D in the treatment of overweight and obese gestational diabetes mellitus patients

Abstract:Objective To explore the clinical efficacy of bifidobacterium combined with vitamin D as an auxiliary treatment for overweight and obese gestational diabetes mellitus.Methods A total of 76 overweight and obese patients with gestational di-abetes who received targeted treatment at Handan Central Hospital from March 2023 to March 2024 were selected and divided into two groups using the random number table method,with 38 cases in each group.The control group received conventional treatment with insulin aspart,while the observation group received conventional treatment with insulin aspart combined with bifidobacterium and vitamin D as an auxiliary treatment.All patients continued the treatment until delivery.The lipid metabolism levels,glucose metabolism levels,pregnancy outcomes,and adverse reactions were compared between the two groups.Results After treatment,the levels of total cholesterol,triglycerides and low-density lipoprotein cholesterol in the observation group were all lower than those in the control group((1.17±0.22)mmol·L-1 vs(1.68±0.35)mmol·L-1,(3.12±1.01)mmol·L-1 vs(3.66±1.10)mmol·L-1,(3.21±0.97)mmol·L-1 vs(3.87±1.05)mmol·L-1,respectively)(t=-7.605,-2.229,-2.846,all P<0.05).After treatment,the levels of fasting blood glucose,glycated hemoglobin,fasting insulin,and insulin resistance index in the observation group were lower than those in the control group((5.13±0.72)mmol·L-1 vs(5.87±0.77)mmol·L-1,(5.03±0.38)% vs(5.28±0.34)%,(11.87±1.35)mU·L-1 vs(15.32±1.42)mU·L-1,(3.14±0.82)vs(3.57±0.93),respectively)(t=-4.327,-3.022,-10.854,-2.138,all P<0.05).The difference in insulin dosage between the two groups was not statistically significant((1,124±23.76)U vs(1,119±26.98)U)(t=0.857,P>0.05).Af-ter treatment,the body mass index of the observation group was lower than that of the control group((24.01±1.28)kg·(m2)-1 vs(24.79±1.69)kg·(m2)-1)(t=-2.268,P<0.05).The incidence rates of adverse pregnancy outcomes(postpartum hemorrhage,premature birth,macrosomia,neonatal asphyxia)in the observation group were all lower than those in the control group(5.26% vs 23.68%,10.53% vs 28.95%,5.26% vs 21.05%,7.89% vs 26.32%,respectively)(χ2=5.208,4.070,4.145,4.547,all P<0.05).The incidence of adverse re-actions in the observation group was lower than that in the control group(13.16% vs 34.21% )(χ2=4.659,P<0.05).Conclusion Bi-fidobacterium combined with vitamin D as an the adjuvant treatment for overweight and obese gestational diabetes mellitus can im-prove glucose and lipid metabolism and reduce the incidence of adverse pregnancy outcomes and adverse reactions.
