Efficacy of repaglinide combined with insulin degludec on treatment of elderly obese patients with type 2 diabetes mellitus

Abstract:Objective To analyze the efficacy of repaglinide combined with insulin degludec in treatment of the elderly obese patients with type 2 diabetes mellitus(T2DM)and observe its impact on glucose and lipid metabolism.Methods A total of 105 elderly obese T2DM patients who were admitted to the Hai'an People's Hospital from January 2022 to December 2023 were en-rolled in the study and randomly divided into two groups.The control group with 52 patients was treated with insulin degludec,while the observation group with 53 patients treated with repaglinide in addition to insulin degludec.Both groups were treated for 12 weeks,and the therapeutic effects were evaluated.Glucose and lipid metabolism(FPG,2 h PG,HbA1c,TC,TG,HDL-C,LDL-C)were detected by an automatic biochemical analyzer,and the levels of these indicators were compared between the two groups before and after the treatment.The incidence rates of adverse reactions were recorded.Results After the treatment,the total ef-fective rate of the observation group(92.45%,49/53)was higher than that of the control group(69.23%,36/52)(χ2=9.180,P<0.05).After 12 weeks of treatment,the FPG,2 h PG,and HbA1c levels of the observation group were respectively(6.24±0.94)mmol·L-1,(7.64±0.92)mmol·L-1,and(6.34±0.83)%,lower than(7.76±1.25)mmol·L-1,(10.22±0.97)mmol·L-1,and(9.12±1.14)% of the con-trol group(t=7.051,13.986,14.305,all P<0.05).After 12 weeks of treatment,the HDL-C level of the observation group was(1.39±0.16)mmol·L-1,higher than(1.17±0.14)mmol·L-1 of the control group(t=7.493,P<0.05).The LDL-C,TC,and TG levels of the observation group were respectively(2.21±0.25)mmol·L-1,(3.45±0.44)mmol·L-1,and(1.96±0.25)mmol·L-1,lower than(3.12±0.37)mmol·L-1,(4.12±0.49)mmol·L-1,and(2.31±0.28)mmol·L-1 of the control group,and there were significant differences(t=14.792,7.375,6.759,all P<0.05).During the treatment period,the incidence of adverse reactions was 9.62% (5/52)in the control group and 11.32% (6/53)in the observation group,and there was no significant difference(χ2=0.081,P>0.05).Conclusion The combination of repaglinide and insulin degludec can effectively improve the clinical efficacy in the treatment of the elderly obese T2DM patients and raise the levels of glucose and lipid metabolism,with the safety favorable.
