Impact of alfacalcidol combined with hemodialysis filtration on hemodynamics,collagen type Ⅳ and toll-like receptor 2 of patients with chronic renal failure

Abstract：Objective To study the impact of alfacalcidol combined with hemodialysis filtration on hemodynamics,collagen type Ⅳ(Col-Ⅳ)and toll-like receptor 2(TLR2)of patients with chronic renal failure.Methods A total of 120 patients with chron-ic renal failure who were hospitalized for targeted therapies in Ningbo Yinzhou No.2 Hospital from August 2022 to November 2024 were enrolled in the study and randomly divided into the control group with 60 cases(treated with hemodialysis)and the observation group with 60 cases(treated with hemodialysis combined with alfacalcidol).The curative effect,the incidence of complications,and the levels of indicators related to bone metabolism,hemodynamics,Col-Ⅳ and TLR2 were observed and compared between the two groups before and after treatment.Results The total effective rate of the observation group was 81.67% (49/60),which was higher than that of 65.00% (39/60)of the control group(χ2=4.261,P<0.05).After treatment,blood calcium(Ca)increased,and blood phos-phorus(P),total parathyroid hormone(iPTH),heart rate,systolic blood pressure,diastolic blood pressure,Col-Ⅳ and TLR2 decreased in both groups.Moreover,the Ca level of the observation group was(2.16±0.37)mmol·L-1,higher than(1.93±0.39)mmol·L-1 of the control group;P,iPTH,heart rate,systolic blood pressure,diastolic blood pressure,Col-Ⅳ and TLR2 of the observation group were respectively(1.48±0.21)mmol·L-1,(125.49±10.77)pg·mL-1,(75.27±3.31)times·min-1,(121.27±3.72)mm Hg(1 mm Hg≈0.133 kPa),(78.89±3.37)mm Hg,(16.93±2.31)ng·mL-1 and(0.89±0.12)μg·mL-1,lower than(1.79±0.10)mmol·L-1,(156.29±10.12)pg·mL-1,(80.77±3.27)times·min-1,(125.27±3.73)mm Hg,(80.02±2.69)mm Hg,(20.61±2.59)ng·mL-1 and(1.23±0.14)μg·mL-1 of the control group,and the differences were statistically significant(t=3.314,10.324,16.143,9.156,5.882,2.030,8.214,14.283,all P<0.05).The incidence of complications in the observation group was 21.67% (13/60)and that in the control group was 36.67% (22/60).There was no significant difference between the two groups(χ2=3.267,P>0.05).Conclusion Alfacalcidol combined with hemodialysis filtra-tion in treatment of patients with chronic renal failure can reduce the levels of Col-Ⅳ,TLR2 and bone metabolism indicators,improve hemodynamics,and have fewer complications.
