Efficacy of roxadustat on correcting anemia of chronic kidney disease patients undergoing dialysis and influence on immune function

Abstract：Objective To evaluate the efficacy and safety of roxadustat in correcting anemia of chronic kidney disease(CKD)patients undergoing dialysis and explore the impact on renal function and immune function.Methods The clinical data were collect-ed from the CKD patients who underwent hemodialysis in Nanyang Central Hospital from February 2020 to December 2022 and were retrospectively analyzed.A total of 120 patients were randomly stratified and divided into the recombinant human erythropoietin(rHuE-PO)group and the roxadustat group according to different treatment methods,with 60 cases in each group.The rHuEPO group received subcutaneous injections of rHuEPO,while the roxadustat group received oral roxadustat capsules.The anemia-related indicators,renal function indicators,erythropoietin(EPO)levels,CD4+/CD8+ratios and anemia improvement were observed and compared between the two groups before and after treatment.Results The anemia-related indicators of hemoglobin,hematocrit,and erythropoietin of the roxadustat group((116.23±10.45)g·L-1,(33.05±5.33)%,(1.46±0.28)μg·L-1)were higher than those in the rHuEPO group((106.76±10.83)g·L-1,(30.77±4.71)%,(0.98±0.26)μg·L-1)after the treatment(t=4.916,2.521,9.786,all P<0.05).The renal function-related indicators of blood urea nitrogen and serum creatinine of the roxadustat group were respectively(21.39±4.33)mmol·L-1 and(853.74±93.21)μmol·L-1,lower than(24.13±5.32)mmol·L-1 and(983.33±99.19)μmol·L-1 of the rHuEPO group(t=3.101,7.755,both P<0.05).The immune indicators of CD4+and CD4+/CD8+ratio of the roxadustat group were respectively(43.05±5.33)% and(1.84±0.21),higher than(35.77±4.71)% and(1.16±0.19)of the rHuEPO group(t=7.984,18.327,both P<0.05).The CD8+level of the roxadustat group was(26.79±5.33)%,lower than(30.14±5.42)% of the rHuEPO group(t=3.427,P<0.05).The anemia improvement rate was 91.67% in the roxadustat group,75.00% in rHuEPO group(χ2=6.000,P=0.014).Conclusion Roxadustat is more effective than rHuEPO in correct-ing anemia of CKD patients undergoing dialysis.As an oral medication,roxadustat provides a convenient and highly compliant treat-ment option,while potentially exerting positive effects on renal and immune functions.
