Abstract:Objective To explore the effect of paliperidone long-acting injection combined with ziprasidone on acute phase and maintenance phase of schizophrenia and its effect on endocrine metabolism. Methods Eighty patients with schizophrenia admitted to Guangyuan Mental Health Center and Chengdu Fourth People's Hospital from January 2021 to August 2023 were se-lected and divided into the research group and control group using the random number table method, with 40 in each group. The patients in the control group received ziprasidone treatment, and those in the experimental group received a combination of ziprasi-done with paliperidone long-acting injectable. The treatment effects, positive and negative symptom scale(PANSS)scores,pre-and post-treatment changes in personal and social functioning, cognitive functioning, and endocrine and metabolic markers were com-pared between the two groups.Meanwhile, the incidence of adverse events was recorded. Results After 12 months of treatment, the total effective rate of the research group was 95.00% (38/40),which was higher than that of the control group 80.00% (32/40)(χ2=4.114,P=0.043).The positive, negative, and PANSS scores of patients in both groups decreased in both the acute and stable phases, with the research group exhibiting lower scores(acute phase:(30.09±2.52)points,(32.46±2.19)points,(66.07±8.54)points;stable phase:(16.08±2.12)points,(15.83±1.01)points,(24.74±6.15)points)as compared with the control group(acute phase:(35.33±3.37)points,(34.45±1.34)points,(73.96±6.18)points;stable phase:(19.27±1.81)points,(16.74±1.56)points,(30.29±5.16)points)(tacute phase=7.876,4.902,4.734,tstable phase=7.238,3.097,4.372,all P<0.05).Additionally,the levels of FBG,TC,TG,and PRL in both groups decreased in both phases, with the research group showing lower levels(acute phase:(4.74±0.15)mmol·L-1,(4.95±0.24)mmol·L-1,(1.42±0.17)mmol·L-1,(13.27±1.69)ng·mL-1;stable phase:(4.51±0.27)mmol·L-1,(4.58±0.35)mmol·L-1,(1.18±0.30)mmol·L-1,(10.68±1.51)ng·mL-1)as compared with the control group(acute phase:(5.69±0.31)mmol·L-1,(5.16±0.15)mmol·L-1,(1.72±0.23)mmol·L-1,(15.23±1.29)ng·mL-1;stable phase:(5.53±0.16)mmol·L-1,(4.83±0.22)mmol·L-1,(1.36±0.14)mmol·L-1,(11.89±1.45)ng·mL-1)(tacute phase=17.447,4.693,6.634,5.831,tstable phase=20.555,3.825,3.439,3.656,all P<0.05).Furthermore,the personal, social, and cognitive function scores of patients in both groups were increased in both phases, with the research group demonstrating higher scores(acute phase:(56.75±3.14)points,(21.55±1.12)points;stable phase:(75.84±3.61)points,(26.18±1.53)points)as compared with the control group(acute phase:(54.57±4.02)points,(20.41±1.68)points;stable phase:(73.16±4.14)points,(24.02±2.11)points)(tacute phase=2.703,3.571,tstable phase=3.086,5.241,all P<0.05).The total incidence of adverse reactions in the research group(15.00%,6/40)was significantly lower than that in the control group(35.00%,14/40)(χ2=4.267,P=0.039).Conclusions Paliperidone long-acting injection combined with ziprasidone is more effective in the acute and mainte-nance phases of schizophrenia since it can improve patients'symptoms, enhance their personal and social functioning as well as cog-nitive functioning, and promote endocrine metabolism levels and has a high degree of safety.
