Efficacy of epalrestat tablets combined with modified qing shang juan tong decoction in treating patients with diabetic peripheral neuropathy

Abstract：Objective To explore the efficacy of modified Qingshang Juantong decoction in treatment of diabetic peripheral neuropathy(DPN)and its influences on glucose metabolism and nerve conduction function.Methods A total of 90 patients with DPN of damp-heat stasis admitted to Suining Municipal Hospital of TCM Affiliated to North Sichuan Medical College from April 2023 to September 2024 were selected and divided into a control group(n=45,treated with epalrestat tablets)and an observation group(n=45,treated with epalrestat tablets+Qingshang Juantong decoction)by random number table method.All patients were treated for 1 month.The clinical curative effect,glucose metabolism indexes(FBG,glycated serum protein(GSP),insulin resistance index(HOMA-IR))and nerve conduction function indicators(motor nerve conduction velocity(MNCV),sensory nerve conduction velocity(SNCV))were compared between the two groups,and the occurrence of adverse reactions was recorded.Results Af-ter treatment,the total effective rate of the observation group was 93.33%(42/45),which was higher than 77.78%(35/45)of the control(χ2=4.406,P<0.05).Before treatment,there was no significant difference in levels of FPG,GSP,HOMA-IR between obser-vation group and control group(all P>0.05).After treatment,the levels of FPG,GSP and HOMA-IR in observation group((8.67±1.88)mmol·L-1,(1.81±0.23)mmol·L-1 and(15.25±2.28))were lower than those before treatment and those in the control((10.01±2.25)mmol·L-1,(1.96±0.25)mmol·L-1,(16.83±3.10))(t=3.066,2.962,2.754,all P<0.05).MNCV and SNCV of median nerve,MNCV and SNCV of common peroneal nerve were higher in the observation group than in the control((48.62±6.08)m·s-1 vs(44.59±5.57)m·s-1,(45.94±5.74)m·s-1 vs(42.37±5.30)m·s-1,(44.46±5.56)m·s-1 vs(41.11±5.14)m·s-1,(42.87±5.36)m·s-1 vs(39.34±4.92)m·s-1),(t=3.279,3.065,2.968,3.255,all P<0.05).There was no significant difference in the incidence of adverse reac-tions(nausea and vomiting)between the observation group(4.44%,2/45)and the control group(6.67%,3/45)(χ2=0.212,P=0.645).Conclusion Epalrestat tablets combined with modified Qingshang Juantong decoction has better improving effect on glucose me-tabolism in DPN,which can improve nerve conduction velocity and has good safety profile.
