Effect of sufentanil combined with etomidate and propofol on fiberoptic bronchoscopy

Abstract：Objective To explore the efficacy and safety of sufentanil combined with etomidate-propofol medium/long-chain fat emulsion mixture in fiberoptic bronchoscopy.Methods A total of 104 patients who underwent elective fiberoptic bronchoscopy due to pulmonary infections in the Hai'an People's Hospital from June 2022 to May 2024 were enrolled in the study and randomly divided into two groups,with 52 cases in each group.The experimental group received etomidate-propofol medium/long chain fat emulsion combined with sufentanil,while the control group received etomidate-propofol medium/long chain fat emulsion combined with fentanyl.The anesthesia effect,vital signs at the time of examination[before entering the scope after induction of anesthesia(T1),when the fiberoptic bronchoscope reached the sound gate(T2),and at the end of the examination(T3)],cognitive function before and after leaving the room,and the incidence rates of adverse reactions during the examination were observed and compared between the two groups.Results The onset of anaesthesia,recovery of consciousness and post-anesthesia care unit stay time of the experi-mental group were respectively[(15.21±4.06)s,(8.12±1.93)min,(20.64±5.18)min],shorter than[(18.37±4.19)s,(10.25±2.41)min,(26.74±6.53)min]of the control group(t=3.906,4.975,5.277,P<0.05).The heart rate and mean arterial pressure at T1 and T3 were respectively[(71.78±9.12)beats·min-1,(85.34±9.73)mmHg,(79.67±9.54)beats·min-1,(89.42±9.56)mmHg]in the experimental group,compared with[(72.12±10.21)beats·min-1,(86.45±10.26)mmHg,(80.21±10.01)beats·min-1,(90.02±9.21)mmHg]in the control group,and there were no significant differences(P>0.05).The heart rate and mean arterial pressure of the two groups were respectively(77.58±9.23)beats·min-1,(89.11±9.42)mmHg,(81.34±9.62)beats·min-1 and(92.87±9.80)mmHg at T2,higher than at T1(t=3.223,2.007,4.740,3.243,P<0.05);the above indexes of the control group were higher than those of the experimental group(t=2.034,1.995,P<0.05).The MMSE scores of the two groups were respectively(27.92±0.84)and(27.65±0.91)after leaving the room,lower than(28.35±0.48)and(28.16±0.53)before the examination(t=3.205,3.492,P<0.05),whereas there were no significant differences in the MMSE scores between the two groups before the examination and after leaving the room(P>0.05).There was no significant difference in the incidence of adverse reactions between the control group(38.46%)and the experimental group(28.85%)during the examination(χ2=1.077,P=0.299).Conclusion The combination of sufentanil and etomidate-propofol medium/long-chain fat emulsion demonstrates superior anesthetic efficacy in fiberoptic bronchoscopy and improves hemodynamic fluctuation with-out affecting the patients'cognitive function,with the safety favorable.
