Assessment of potentially inappropriate medication among elderly patients treated with multiple medications based on Beers Criteria 2023

Abstract:Objective To explore the influencing factors for potentially inappropriate medication among elderly patients treat-ed with multiple medications so as to facilitate the reasonable clinical use of drugs.Methods The medical records were collected from the elderly patients who were hospitalized in the Fuyang People's Hospital from June to September 2023.Potentially inappro-priate medication(PIM)was evaluated based on the Beers Criteria 2023.Results Among the 571 patients included,390(68.30%)had at least one PIM,with a total of 1 045 PIM occurrences.These included 413 case-times of medications to avoid,42 case-times of disease/syndrome-related PIM,527 case-times of medications requiring caution,13 case-times of drug interaction-related PIM,and 50 case-times of PIM associated with renal impairment in the elderly.Multivariate Logistic regression analysis showed that the independent risk factors for PIM in elderly polypharmacy patients included the number of medications(OR:0.789,95%CI:(0.728,0.856),P<0.05),the number of diagnosed diseases(OR:0.906,95%CI:(0.818,0.994),P<0.05),comorbid type 2 diabetes(OR:0.522,95%CI:(0.287,0.949),P<0.05)and heart failure(OR:0.210,95%CI:(0.098,0.453),P<0.05).Conclusion PIM is prevalent among elderly polypharmacy patients.Clinical pharmacists can utilize the Beers Criteria 2023 to develop feasible treatment plans,enhance medication safety,and further improve rational drug use in elderly patients.
