Effect of menaquinone-4-assisted internal fixation on treatment of elderly patients with osteoporosis intertrochanteric fractures

Abstract：Objective To explore the efficacy of menaquinone-4-assisted internal fixation in treatment of elderly patients with osteoporosis intertrochanteric fractures.Methods A total of 80 elderly patients with intertrochanteric fractures due to osteoporosis who were admitted to the Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine from April 2022 to September 2023 were randomly divided into the control group and the observation group(40 cases each).Both groups were treated with prox-imal femoral square rotation intramedullary nail internal fixation,and after the operation the control group was given calcitriol soft capsules and calcium carbonate D3 tablets.The observation group was treated with menaquinone-4 on the basis of control.The clinical efficacy,changes in bone metabolism,hip joint function,pain and bone density before and after treatment,adverse drug reactions,and the risk of fractures at the original site were observed and compared between the two groups.Results The total effective rate of the observation group was 97.50%(39/40),higher than 75.00%(30/40)of the control group(χ2=8.538,P<0.05).The hip joint function score(HHS)of the observation group was((85.04±4.90)points)after the treatment,higher than((75.71±4.85)points)of the control group.The visual analogue scale(VAS)of the observation group was((1.02±0.47)points),lower than((1.58±0.64)points)of the control group,and there was significant difference(t=8.559,4.460,both P<0.001).The levels of serum type I procollagen N-terminal propeptide(PINP)and N-terminal osteocalcin(N-MID)of the observation group were respectively((36.21±5.69)ng·mL-1,(32.16±9.36)ng·mL-1 and(0.64±0.04)g·cm-2 after the treatment,higher than((32.00±4.36)ng·mL-1,(26.88±8.00)ng·mL-1 and(0.59±0.06)g·cm-2)of the control group,and there were significant differences(t=3.714,2.712,4.385,all P<0.05).The level of β-CrossLaps of type I Collagen(β-CTX)of the observation group was((0.42±0.08)pg·mL-1)after the treatment,lower than((0.50±0.10)pg·mL-1)of the control group,and there was significant difference(t=3.951,P<0.001).The incidence of adverse reactions was 2.50%(1/40)in the observation group,5.00%(2/40)in the control group,and there was no significant difference(χ2=0.000,P=1.000).The recurrence rate of fractures at the original site of the observation group was 2.50%(1/40),lower than 20.00%(8/40)of the control group(χ2=6.135,P<0.05).Conclusion The clinical efficacy of menaquinone-4-assisted internal fixation in the treatment of intertrochanteric fractures in elderly patients with osteoporosis is definite.It promotes postoperative fracture healing,effectively improves bone metabolism and osteoporosis,reduces the risk of re-fracture,and has high medication safety.
