Impact of Bacillus Licheniformis Granules,Live combined with enteral nutrition support on intestinal flora and immune microenvironment of children with severe pneumonia

Abstract:Objective To explore the effects of Bacillus Licheniformis Granules,Live combined with enteral nutrition support on the intestinal flora and immune microenvironment of children with severe pneumonia.Methods A total of 118 children with severe pneumonia who visited the pediatrics department of Nanyang Second General Hospital from July 2021 to May 2023 were selected as the research subjects and retrospectively analyzed.They were divided into the intervention group(n=66)and the con-trol group(n=52)according to different treatment methods.The control group was given basic symptomatic treatment and enteral nutrition support,and the intervention group was given extra Bacillus Licheniformis Granules,Live on the basis of the control group.The clinical efficacy,changes in intestinal flora,changes in serum inflammatory factors,intestinal barrier infection status,changes in nutritional indicators,as well as survival rates and complication status were compared between the two groups.Results The clinical effective rate of the intervention group was 92.42%,higher than 76.92%of the control group(χ2=5.667,P=0.017).After the treatment,the numbers of Lactobacillus,Bacillus licheniformis,and Bifidobacterium breve in the intestines of the intervention group were(9.86±3.67)log CFU·g-1,(10.10±1.20)log CFU·g-1 and(11.50±3.58)log CFU·g-1 respectively,higher than(8.89±2.34)log CFU·g-1,(9.20±2.54)log CFU·g-1 and(9.66±3.20)log CFU·g-1 of the control group.The numbers of Clostridium hathewayi,Actinomy-ces viscosus,and Enterococcus in the intervention group were(7.65±2.32)log CFU·g-1,(7.25±1.61)log CFU·g-1 and(7.04±2.68)log CFU·g-1 respectively,lower than(8.65±3.01)log CFU·g-1,(8.20±1.54)log CFU·g-1 and(7.95±2.20)log CFU·g-1 in the con-trol group.The differences were statistically significant(t=2.159,2.543,2.903,2.038,3.243,1.998,all P<0.05).The levels of PCT,endotoxin and DAO in serum in the intervention group were(1.34±0.67)ng·mL-1,(0.14±0.20)EU·mL-1 and(1.80±3.68)U·mL-1 re-spectively,lower than(2.68±0.66)ng·mL-1,(0.35±0.14)EU·mL-1 and(3.41±3.20)U·mL-1 in the control group(t=10.857,6.429,2.497,all P<0.05).The levels of IL-10,IL-4,and IFN-γ in the intervention group were(15.12±5.21)pg·mL-1,(4.10±1.20)pg·mL-1 and(8.32±3.98)pg·mL-1 respectively,lower than(18.87±4.33)pg·mL-1,(5.20±1.54)pg·mL-1 and(13.50±2.66)pg·mL-1 in the control group(t=4.176,4.362,6.621,all P<0.05).The levels of ALB,HGB,and the nitrogen balance index in the intervention group were(40.20±8.45)g·L-1,(165.20±22.54)g·L-1 and(8.50±5.98)g·d-1 respectively,higher than(30.45±7.68)g·L-1,(134.10±30.20)g·L-1 and(2.41±6.20)g·d-1 in the control group(t=6.550,5.680,5.404,all P<0.05).The comparison of long-term survival rates between the two groups showed no statistically significant difference(χ2=0.516,P=0.472).The incidence of diarrhea of the intervention group was 10.61%,lower than 23.08%of the control group(χ2=5.253,P=0.022)and the incidence of respiratory failure of the intervention group was 9.09%,lower than 21.15%of the control group(χ2=10.109,P=0.001);there was statistically significant difference between the two groups.Conclusion Bacillus Licheniformis Granules,Live combined with enteral nutrition support can improve the intestinal microenvironment and pulmonary immune microenvironment of children,and increase the cure rate of severe pneumonia.
