Efficacy of Lactobacillus combined with fluconazole in treatment of patients with recurrent mycosis vaginitis

Abstract：Objective To observe the efficacy and safety of Lactobacillus combined with fluconazole in the treatment of patients with recurrent mycosis fungoides and analyze the recurrence rate.Methods The clinical data were collected from 112 patients with recurrent mycosis vaginitis who were admitted to Yuncheng Central Hospital Affiliated to Shanxi Medical University from July 2021 to June 2024 and the enrolled patients were divided into the combination group with 56 cases(treatment with fluconazole capsules combined with Lac-tobacillus preparation Yanhua capsule)and the single-medicine group with 56 cases(treatment with fluconazole capsules)according to the different treatment regimens.The clinical efficacy,the time of clinical symptom relief,the change of vaginal microecological environment,the level of inflammatory factors and the recurrence rate were observed and compared between the two groups.Results The total clinical effectiveness rate of the combination group was 96.43%(54/56)after the treatment,higher than 83.93%(47/56)of the single-drug group(χ2=4.940,P<0.05).The disappearance time of the symptoms of lumbar-abdominal pain,frequent urination and painful urination,and vulvar itching and redness and swelling and the time for leukorrhea to return to normal of the combination group were respectively((6.59±1.02)d,(7.12±1.05)d,(6.54±0.91)d,(5.78±0.80)d),shorter than(8.17±1.24)d,(8.28±1.17)d,(7.86±1.13)d,and(6.65±0.95)d of the single-drug group(t=7.364,5.522,6.808,5.242,all P<0.05).The pH of the combination group was(5.53±0.07)after the treatment,lower than(5.64±0.08)of the monotherapy group,and the bacterial flora density was improved more remarkably in the combination group than in the monotherapy group(t=7.744,Z=2.143,both P<0.05).The tumor necrosis factor(TNF-α)and interleukin-6(IL-6)levels of the combination group were respectively((2.36±0.42)μg·L-1 and(36.54±4.62)μg·L-1),lower than(3.72±0.61)μg·L-1 and(52.38±5.71)μg·L-1 of the single-drug group(t=13.742,16.138,both P<0.05).At 3 and 6 months of follow-up,the disease recurrence rates of the combination group were respectively 5.36%(3/56)and 8.93%(5/56),lower than 17.86%(10/56)and 23.21%(13/56)of the single-drug group(χ2=4.264,4.236,both P<0.05).Conclusion Lactobacillus combined with fluconazole can achieve remarkable clinical effect on treatment of patients with recur-rent mycosis vaginitis,accelerate the improvement of clinical symptoms or signs,optimize the vaginal microecological environment,and reduce the levels of inflammatory factors,with the recurrent rate low.
