Efficacy of low molecular weight heparin sodium combined with dydrogesterone in treatment of patients with threatened abortion complicated with subchorionic hematoma

Abstract:Objective To investigate the effectiveness of low molecular weight heparin sodium combined with dydrogesterone in treating threatened abortion(TA)complicated with subchorionic hematoma(SCH).Methods A total of 98 patients with TA complicated with SCH who were admitted to the Department of Obstetrics and Gynecology of the First Affiliated Hospital of Henan University of Science and Technology from March 2023 to March 2024 were selected and divided into two groups,with 49 cases in each group by using the random number table method.The control group received oral folic acid and dydrogesterone treatment,while the observation group received subcutaneous injection of low molecular weight heparin sodium combined with oral adminis-tration of folic acid and dydrogesterone.The efficacy,reproductive hormone levels,immune function,coagulation function,adverse reaction rates and pregnancy outcomes were compared between the two groups.Results The total effective rate in the observa-tion group[87.76%(43/49)]was higher than that in the control group[67.35%(33/49)](χ2=5.861,P=0.015).After treatment,the levels of progesterone and β-human chorionic gonadotropin(β-hCG)in both the control group[(37.65±4.69)ng·mL-1,(14 337.42±556.81)mIU·mL-1]and the observation group[(44.15±4.22)ng·mL-1,(18 295.89±423.56)mIU·mL-1]were higher than those before treatment[(15.01±2.77)ng·mL-1,(9 213.97±255.18)mIU·mL-1;(14.23±2.89)ng·mL-1,(9 245.89±267.98)mIU·mL-1],and the levels of these indexes in the observation group were obviously higher than in the control group(t=7.212,39.607,both P<0.05).After treat-ment,the levels of tumor necrosis factor-alpha(TNF-α)and interferon-gamma(IFN-γ)in both the control group[(13.01±2.33)pg·mL-1,(10.12±2.17)pg·mL-1]and the observation group[(8.27±2.12)pg·mL-1,(6.01±1.44)pg·mL-1]were lower than before treatment[(28.94±3.02)pg·mL-1,(18.04±2.78)pg·mL-1,(29.56±3.14)pg·mL-1,(17.54±2.25)pg·mL-1],and the levels of these indexes in the observation group were obviously lower than in the control group(t=10.533,11.047,both P<0.05).There were no statistically signif-icant differences in anti-Xa activity,activated partial thromboplastin time(aPTT)and platelet count before and after treatment,or be-tween the two groups after treatment(all P>0.05).The total incidence of adverse reactions was 0(0/49)in the control group and 2.04%(1/49)in the observation group,with no significant difference between the two groups(χ2=0.000,P=1.000).After treatment,the full-term delivery rate in the observation group was 79.59%(39/49),higher than 57.14%(28/49)in the control group,with statistically significant difference(χ2=5.709,P=0.017).The live birth rate in the observation group was 93.88%(46/49),higher than 77.55%(38/49)in the control group,with statistically significant difference(χ2=5.333,P=0.021).Conclusion Low molecular weight heparin sodi-um combined with dydrogesterone in treating TA complicated with SCH can enhance efficacy,regulate immune function and repro-ductive hormone levels,improve pregnancy outcomes,and demonstrate great safety.
