Application and safety of remimazolam combined anesthesia in patients undergoing upper airway surgery

Abstract:Objective To explore the effect and safety of remifentanil-combined anesthesia in upper airway surgery.Methods A total of 108 patients receiving upper airway surgery in Branch of Huai'an First People's Hospital from August 2023 to August 2024 were selected as the research subjects and divided into the control group and the observation group using the meth-od of randomized numerical table,with 54 in each group.Anesthesia in the control group was induced with propofol,while that in the observation group was induced with remifentanil.The hemodynamic indicators before anesthesia induction(T0),at the start of surgery(T1),at 15 minutes of surgery(T2),and at the end of surgery(T3),the analgesic and sedative effects and the occurrence of adverse reactions at 2 hours and 12 hours after surgery were compared between the two groups.Results The mean arterial pres-sure(MAP)at T1,T2,and T3 in the control group((77.03±7.33)mm Hg,(76.98±8.23)mm Hg,(78.93±8.33)mm Hg(1 mm Hg≈ 0.133 kPa))were lower than that at T0((82.54±7.61)mm Hg)(t=﹣3.832,﹣3.645,﹣2.950,all P<0.05),and the blood oxygen saturation at T1,T2,and T3((95.14±3.13)%,(95.03±3.17)%,(94.92±3.31)%)were lower than that at T0(97.47±2.72)%(t=4.129,4.293,4.374,all P<0.05).The heart rate(HR)at T1 and T2((70.03±8.97)times·min-1,(74.15±8.23)times·min-1)were lower than that at T0(77.47±7.53)times·min-1(t=4.668,2.187,both P<0.05).The MAP of the observation group at T1,T2 and T3 were higher than that of the control group((80.07±8.15)mm Hg vs(77.03±7.33)mm Hg,(81.15±8.54)mm Hg vs(76.98±8.23)mm Hg,(81.23±8.12)mm Hg vs(78.93±8.33)mm Hg(t=-2.038,-2.584,-2.034,all P<0.05)),and the HR at T1 and T2 were higher than that of the control group((75.07±8.12)times·min-1 vs(70.03±8.97)times·min-1,(77.49±8.54)times·min-1 vs(74.15±8.23)times·min-1(t=-3.061,-2.069,both P<0.05)).The blood oxygen saturation at T3 was higher than that of the control group((96.78±3.65)%vs(94.92±3.31)%(t=﹣2.774,P<0.05)).The VAS scores of patients in the observation group were lower than those in the control group 2 hours and 12 hours after surgery((4.55±0.78)points vs(5.01±0.97)points,(3.59±0.91)points vs(4.15±0.87)points)(t=2.716,3.269,both P<0.05).There was no significant difference in Ramsay scores between the two groups of patients(P>0.05).As compared with the control group,the sed-ative onset time and recovery time in the observation group was shorter((58.74±4.44)s vs(71.74±5.42)s,(9.83±1.78)min vs(12.61±2.04)min(t=13.635,7.546,all P<0.05)).Meanwhile,the incidence rate of adverse reactions in the observation group was lower(7.41%vs 27.78%)(χ2=7.728,P<0.05).Conclusion Remimazolam exerts little impact on hemodynamics but has good analgesic effect,fast onset,high metabolism speed and is of good safety.
