Application of ursodeoxycholic acid and somatostatin in perioperative period of cholecystectomy for gallstone cholecystitis

Abstract:Objective To explore the effect of perioperative use of ursodeoxycholic acid capsules combined with somatostatin on the surgical-related indicators and inflammation level in patients undergoing laparoscopic cholecystectomy for calculous chole-cystitis.Methods A total of 96 patients with calculous cholecystitis undergoing laparoscopic cholecystectomy in Jiaozuo People's Hospital from January 2023 to May 2024 were selected and randomly divided into the control group(n=48)and observation group(n=48)by random number table method.In the perioperative period,the patients in the control group were treated with ursodeoxy-cholic acid capsules while those in the observation group with ursodeoxycholic acid capsules combined with somatostatin.The surgi-cal-related indicators,inflammatory factor levels at admission and after surgery,and liver function indicators,the incidence of adverse reactions were compared between the two groups.Results The operation time and postoperative hospitalization time of the pa-tients was shorter in the observation group than in the control((45.13±8.43)min vs(56.75±10.12)min,(5.19±0.82)d vs(7.12±0.54)d)and the amount of intraoperative bleeding was less in the observation group than in the control((42.72±13.04)mL vs(70.59±17.41)mL)(t=6.112,13.619,8.877,all P<0.05).On the 5th day after surgery,the levels of high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the observation group((6.42±1.37)mg·L-1 vs(22.71±5.06)mg·L-1,(38.57±9.45)ng·L-1 vs(182.42±25.36)ng·L-1,(0.75±0.12)μg·L-1 vs(2.13±0.23)μg·L-1)and the control group((10.87±3.28)mg·L-1 vs(22.82±4.74)mg·L-1,(52.23±14.76)ng·L-1 vs(181.47±24.39)ng·L-1,(1.61±0.18)μg·L-1 vs(2.11±0.21)μg·L-1)were lower than those on admission,and they were lower in the observation group than in the control(t=8.673,5.400,27.542,all P<0.05).On the 5th day after surgery,the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL)of the patients in the observation group((38.37±6.81)U·L-1 vs(103.72±19.31)U·L-1,(38.78±7.54)U·L-1 vs(82.75±10.14)U·L-1,(23.89±1.43)μmol·L-1 vs(59.65±11.27)μmol·L-1)and the control group((56.19±8.42)U·L-1 vs(102.34±18.45)U·L-1,(57.61±8.33)U·L-1 vs(83.24±10.07)U·L-1,(31.38±3.79)μmol·L-1 vs(59.24±12.53)μmol·L-1)were all lower than those on admission,and the levels of prealbumin(PA)levels((287.51±29.31)mg·L-1 vs(165.75±22.24)mg·L-1),((226.35±22.57)mg·L-1 vs(171.56±20.37)mg·L-1)were higher than those on admission,and the ALT,AST,and TBIL levels of observation group were lower than those of control group,and the PA level was higher than that of the control group(t=11.401,11.611,12.810,11.454,all P<0.05).The differ-ence was not significant in the total incidence of adverse reactions between the two groups(12.50%vs 10.42%,χ2=0.103,P=0.749).Conclusion Ursodeoxycholic acid capsules combined with somatostatin can shorten the surgical time and hospitalization time of patients undergoing laparoscopic cholecystectomy for calculous cholecystitis.Meanwhile,it can reduce the levels of inflammatory factors and protect the liver function of patients and has good safety profile.
