Clinical evaluation of osimertinib in combination with pemetrexed and csplatin in treatment of patients with EGFR-mutated lung adenocarcinoma

Abstract:Objective To explore the clinical efficacy and adverse reactions of osimertinib combined with chemotherapy in treatment of lung adenocarcinoma with epidermal growth factor receptor(EGFR)mutation.Methods Ninety-eight patients with epidermal growth factor receptor mutation lung adenocarcinoma admitted to Anhui Jimin Tumor Hospital from July 2022 to Decem-ber 2023 were included in this study,and divided into the experimental group(n=49,osimertinib combined with pemetrexed and cisplatin chemotherapy)and the control group(n=49,pemetrexed combined with cisplatin chemotherapy)using a randomized nu-merical table method.After 4 courses of treatment,the clinical efficacy,serum tumor marker levels,immune function and incidence of adverse reactions were compared between the two groups.Results The objective remission rate of drugs in the experimental group after treatment was 69.39%(34/49),which was higher than that of 44.90%(22/49)in the control(χ2=6.000,P<0.05),and the disease control rate of the two groups was 97.96%and 89.80%,respectively,with the difference being not statistically significant(χ2=1.598,P>0.05).After treatment,the serum levels of carcinoembryonic antigen,glycan antigen 125,glycan antigen 199,and cyto-keratin 19 fragment antigen 21-1 in the experimental group((9.17±2.34)ng·mL-1,(49.85±6.81)U·mL-1,(53.31±9.77)U·mL-1,(1.77±0.66)μg·mL-1,respectively)were all lower than those in the control((14.03±3.04)ng·mL-1,(64.23±7.95)U·mL-1,(59.59±10.03)U·mL-1,and(3.18±0.61)μg·mL-1,respectively)(t=8.868,9.620,3.140,and 10.982,all P<0.05).Meanwhile,the levels of antigenic CD3+,CD4+and CD4+/CD8+in the experimental group((67.68±6.40)%,(36.15±5.71)%,(1.44±0.48),respectively)were higher than those in the control((59.75±5.37)%,(29.53±3.55)%,(1.25±0.29),respectively)(t=6.644,7.389,2.372,all P<0.05).The difference in the total incidence of adverse reactions was not statistically significant between the experimental group and the control group(24.49%vs 18.37%,χ2=0.545,P>0.05).Conclusion Chemotherapy using osimertinib combined with chemotherapy can effectively improve the clinical efficacy of patients with EGFR-mutation lung adenocarcinoma because it can regulate the level of serum tumor markers and immune function.Furthermore,it is of high safety,and has clinical application value.
