Clinical evaluation of tislelizumab combined with pemetrexed and carboplatin in treatment of patients with non-small cell lung cancer

Abstract:Objective To investigate the efficacy of tislelizumab combined with pemetrexed and carboplatin in the treatment of stage Ⅲb-Ⅳ non-small cell lung cancer(NSCLC).Methods From April 2021 to April 2024,80 patients with NSCLC at Zhengzhou Central Hospital were selected and divided into control group and study group by computer-generated random number table method,and 40 patients in each group were observed.The patients in the control group were given pemetrexed and carbo-platin chemotherapy,while those in the study group were treated with tirelizumab on this basis.The two groups were compared in terms of clinical efficacy,inflammatory reaction,immune function,tumor marker levels and the occurrence of adverse reactions.Results After 9 weeks of treatment,the objective response rate and disease control rate in the study group(60.00%and 90.00%respectively)were higher than those in the control(35.00%and 67.50%respectively),showing statistical significance(χ2=5.013,6.050;both P<0.05).The levels of platelet/lymphocyte,neutrophil/lymphocyte,systemic immune inflammatory index and CD8+in the study group were(111.75±29.83),(2.03±1.12),(129.49±23.16)and(22.51±2.39)%respectively,which were lower than those in the control((137.51±34.48),(2.76±1.29),(204.11±30.25),(24.66±2.78)%)(t=3.573,2.703,12.388,3.709;all P<0.05).CD4+and CD4+/CD8+levels were(42.85±4.06)%and(1.92±0.27)in the study group and they were higher than those in the control((35.26±3.73)%and(1.44±0.19))(t=8.707,9.195;both P<0.05).The levels of cytokeratin 19 fragment antigen 21-1,carbohydrate antigen 199 and squa-mous cell carcinoma antigen in the study group were(4.65±1.08)ng·mL-1,(37.84±4.66)U·mL-1 and(19.05±2.42)ng·mL-1 and they were lower than those in the control((7.21±1.53)ng·mL-1,(45.79±5.43)U·mL-1,(27.14±3.29)ng·mL-1)(t=8.645,7.027,12.528;all P<0.05).In the study group,the overall rate of adverse reactions(12.50%)was reduced as compared with the control group(35.00%)(χ2=5.591,P<0.05).Conclusion For patients with stage Ⅲb-Ⅳ NSCLC,tirelizumab combined with pemetrexed and carboplatin chemotherapy regimen has a definite effect since it can improve the inflammatory response,promote the immune function,reduce the level of serum tumor markers,and has a high safety profile.
