Analysis of 103 cases of adverse reactions due to sodium valproate in treatment of post-stroke epilepsy

Abstract:Objective To analyze the adverse drug reactions(ADRs)in 103 post-stroke epileptic patients treated with sodi-um valproate to explore the characteristics of its occurrence and clinical coping strategies to provide reference for the rational use of drugs in the clinic.Methods The clinical data of 103 patients with post-stroke epilepsy who developed ADRs after receiving treatment with sodium valproate in Kashgar Prefecture Second People's Hospital were retrospectively analyzed from January 2021 to May 2024.The included patients were statistically analyzed for their gender,age,administered dose,blood concentration,involved organs/systems,and ADR management results.Results The 103 patients with post-stroke epilepsy who developed ADRs,a total of 137 ADRs were reported,and the main organs/tissues involved were the digestive system,the nervous system and the haematological system,with a total of 96 cases(71.11%).The blood drug concentration was below the therapeutic window in 20 cases(19.42%)and beyond the therapeutic window in 28 cases(27.18%),and the probability of serious ADRs in such patients was 14.58%(7/48),which was higher than that of the patients in the effective therapeutic range of 3.64%(2/55),showing no statistically significant difference(P>0.05).A total of 68 patients(66.02%)were treated symptomatically with drug reduction or stopping.Most of the patients'ADRs were under good control.Treatment,drug dosage reduction or drug discontinuation,and ADRs were well controlled in most patients,among which 9 patients(8.74%)discontinued the drug due to severe ADRs and switched to other antiepileptic drug treatment.Over-all,98(95.15%)patients had good ADR outcomes.Conclusion Although sodium valproate has a good safety profile,its possible adverse effects will still affect the treatment process of the disease.Therefore,in the clinical application of sodium valproate,the de-tection of its ADRs should be strengthened,and the treatment programme should be adjusted in a timely manner to ensure the safety of the drug.
