Efficacy of simethicone combined with dagliflozin and its short-term prognostic impact on type 2 diabetes mellitus with cardiorenal insufficiency

Abstract:Objective To investigate the efficacy of simethicone combined with dagliflozin and its short-term prognostic im-pact on patients with type 2 diabetes mellitus complicated with cardiac and renal insufficiency.Methods A total of 103 patients with type 2 diabetes mellitus complicated with cardiac and renal insufficiency admitted to Anqing Second People's Hospital from March 2022 to June 2024 were selected as the research subjects and randomized into a study group(n=52)and a control group(n=51).The pa-tients in the control group received dagliflozin in combination with conventional treatment and those in the study group were treated with simethicone in combination with simethicone based on the regimen in the control group.The blood glucose level,cardiac and renal func-tion indexes and safety before and after the treatment were compared between the two groups.Results After treatment,the glycosylated haemoglobin,fasting blood glucose and 2-hour postprandial blood glucose levels in the study group((6.42±1.34)%,(7.06±1.35)mmol·L-1,(8.67±1.74)mmol·L-1)were lower than those in the control((7.36±1.57)%,(8.39±1.51)mmol·L-1,(9.91±1.58)mmol·L-1)(t=3.271,4.715,3.784,all P<0.05).The levels of left ventricular ejection fraction,output per beat and glomerular filtration rate in the study group were higher than those in the control after treatment((51.36±3.17)%vs(47.69±3.42)%,(65.19±4.31)mL vs(61.27±5.27)mL,(92.78±10.11)mL·min-1·1.73 m-2 vs(86.15±9.67)mL·min-1·1.73 m-2)(t=5.650,4.136,3.400,all P<0.05).The levels of plasma amino-terminal brain natriuretic peptide precursor,blood creatinine and urea were lower in the study group than in the control((435.62±104.17)ng·L-1 vs(493.52±95.39)ng·L-1,(78.23±10.54)μmol·L-1 vs(86.45±9.71)μmol·L-1,(5.36±1.24)mmol·L-1 vs(6.21±1.17)mmol·L-1)(t=2.940,4.114,3.577,all P<0.05).A total of 10(19.61%)adverse reactions occurred in the study group,which was lower than the 20(38.46%)in the control(χ2=4.434,P<0.05).The incidence of MACE in the study group was 3.85%(2/52),which was lower than that of 17.65%(9/51)in the control(χ2=5.414,P=0.023).Conclusion Simeglutide combined with dagliflozin can effectively control blood glucose levels,significantly improve cardiac and renal function and reduce the incidence of short-term adverse cardiovascular events in patients with T2DM complicated with cardiac and renal insufficiency.Meanwhile,it is of good clinical efficacy and safety.
